探讨纤维支气管镜对机械通气治疗COPD并呼吸衰竭的临床效果及安全性

目的探究纤维支气管镜对机械通气治疗COPD并呼吸衰竭的临床效果及安全性。方法将我院2013年2月~2015年2月收治的58例COPD合并呼吸衰竭患者,随机分为观察组和对照组,各29例。对比两组血气分析结果、机械通气时间、住院时间、临床疗效。结果治疗4 h,两组Pa O2、Pa CO2、Sa CO2均优于治疗前,且观察组改善效果更明显。两组机械通气、住院、VAP治愈时间、首次撤机成功率和VAP发生率、疗效比较,P〈0.05。结论纤维支气管镜对机械通气治疗COPD合并呼吸衰竭的疗效有积极作用,且有效控制局部感染,减少机械通气时间。

Objective To explore the clinical effect and safety of fiber bronchoscopy in the treatment of COPD and respiratory failure in patients with mechanical ventilation. Methods 58 cases of COPD with respiratory failure in our hospital from February 2013 to February were randomly divided into observation group and control group, 29 cases in each group. Blood gas analysis results, mechanical ventilation time, hospital stay, clinical efficacy of the two groups were compared. Results Treatment of 4 h, the two groups of Pa O2, Pa CO2, Sa CO2 were better than before treatment, and the observation group to improve the effect is more obvious. Two groups of mechanical ventilation, hospitalization, VAP cure time, the first weaning success rate and the incidence of VAP, efficacy comparison, P〈0.05. Conclusion The efficacy of mechanical ventilation in the treatment of COPD combined with respiratory failure has a positive effect on the treatment of the patients with mechanical ventilation, and it can effectively control the local infection and reduce the time of mechanical ventilation.
