Preoperative and Postoperative Physical Conditions of Patients with Thyroid Cancer

目的:探讨甲状腺癌患者术前、术后的健康状态。方法:采用症状自评量表(SCL-90),对60例甲状腺癌患者术前、术后进行心率、血压和心理进行测查,并与正常人群对照。结果:1术前心率、收缩压、舒张压得分病例组高于对照组(t=8.569,5.616,5.177;P<0.01),术后心率、舒张压得分病例组高于对照组(t=5.244,2.329;P<0.05);2术前抑郁、焦虑、恐怖因子得分病例组高于对照组(t=23.913,25.430,22.269;P<0.01),术后躯体化、强迫、人际敏感、抑郁、焦虑、敌对、恐怖、偏执因子得分病例组高于对照组(t=15.637,4.346,8.261,35.581,38.738,11.704,48.345,7.645;P<0.01);3病例组心率、收缩压、舒张压得分术前高于术后(t=8.234,7.107,6.152;P<0.01);4病例组躯体化、强迫、人际敏感、抑郁、焦虑、敌对、恐怖、偏执因子得分手术后高于手术前(t=-30.168,-10.379,-18.156,-14.221,-19.634,-19.077,-16.634,-21.645;P<0.01)。结论 :甲状腺癌患者术前、术后存在明显生理和心理问题,需进行心理干预治疗。

Objective：To investigate the preoperative and postoperative physical conditions of patients with thyroid cancer.Methods：We examined both the preoperative and postoperative heart rate,blood pressure and mental state of60 patients according to Symptom Checklist 90（SCL-90）,and made a contrast to the normal people.Results：1The thyroid cancer patients＇ preoperative heart rate,blood pressure,systolic pressure and diastolic blood pressure scores were higher in case group than control group（t=8.569,5.616,5.177;P〈0.01）.The thyroid cancer patients＇ postoperative heart rate and diastolic blood pressure scores were higher in case group than control group（t=5.244,2.329,P〈0.05）;2 The preoperative SCL-90 total score,depression,anxiety,terror factor scores were higher in case group than control group（t=-6.586,23.913,25.430,22.269;P〈0.01）;The postoperative SCL-90 total score,somatization,compulsion,interpersonal sensitivity,depression,anxiety,hostile,terror,paranoid factor scores were higher in case group than control group（t=19.957,15.637,4.346,8.261,35.581,38.738,11.704,48.345,7.645;P〈0.01）;3In case group,the heart rate,systolic pressure and diastolic blood pressure scores were higher in preoperative than that in postoperative（t=8.234,7.107,6.152;P〈0.01）;4In case group,the SCL-90 total score,somatization,compulsion,interpersonal sensitivity,depression,anxiety,hostile,terror,paranoid factor scores were higher in preoperative than postoperative（t=-35.322,-30.168,-10.379,-18.156,-14.221,-19.634,-19.077,-16.634,-21.645;P〈0.01）.Conclusion：There are obvious physiological and psychological problems with thyroid cancer patients before and after having operations.They need psychological intervention treatments.
