循环保护性通气策略的提出与内涵 被引量:15

摘要 肺保护性通气策略已成为目前治疗严重低氧和急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)的基本原则[1],其主要目的是避免压力或容积对肺组织的过度牵拉而造成肺损伤,进而避免远隔器官损害.近年来值得注意的是,越来越多的关于ARDS的研究发现,ARDS患者并非死于严重肺损伤和顽固性低氧血症,而是死于休克和突发心跳骤停[2].因此,在当前的机械通气策略中仅强调肺保护避免肺损伤可能还远远不足.临床医疗及科研不应仅关注呼吸力学对肺脏的影响,更应关注呼吸运动产生的血流动力学效应[3].在开放肺、保护肺的同时应避免机械通气对循环的损伤,进而达到改善患者预后的目的.
