IL-5及其受体IL-5Rα在慢性鼻-鼻窦炎伴息肉患者息肉组织中的表达及意义

Expression and its significance of IL-5 and its receptor IL-5Rα in nasal polyp of patients with chronic rhinosinusitis

目的探讨IL-5及其受体IL-5Rα在慢性鼻-鼻窦炎伴息肉(CRSw NP)患者息肉组织中的表达及意义。方法 89例CRSw NP患者根据嗜酸性粒细胞在摘除的鼻息肉组织中的浸润程度将患者分为ECRSw NP组(n=41)和对照组(n=48),收集患者一般临床资料,利用HE染色计数鼻息肉组织中嗜酸性粒细胞表达,利用RTPCR技术检测IL-5和IL-5Rα在鼻息肉组织中表达。结果 ECRSw NP组患者变应性鼻炎史比例、双侧鼻息肉比例、鼻窦累及数、鼻息肉病变程度评分和Lund-Mackay评分均高于对照组,差异均有统计学意义(P<0.05),ECRSw NP组患者嗜酸性粒细胞计数、嗜酸性粒细胞百分比和总Ig E均大于对照组,差异均有统计学意义(P<0.05);ECRSw NP组患者鼻息肉组织中每个高倍视野嗜酸性粒细胞平均计数为(36.5±14.2),远高于对照组患者(1.2±0.6),差异具有统计学意义(t=17.146,P=0.000),Pearson相关分析显示,ECRSw NP组患者鼻息肉组织中嗜酸性粒细胞计数与生化指标中血嗜酸性粒细胞计数和百分比均呈正相关(r=0.703和0.652,均P<0.05);ECRSw NP组患者鼻息肉组织中IL-5 mRNA和IL-5RαmRNA相对表达量均高于对照组,差异均有统计学意义(P<0.05),Pearson相关分析显示,ECRSw NP组患者鼻息肉组织中IL-5 mRNA相对表达量与IL-5RαmRNA相对表达量呈正相关(r=0.454,P=0.003)。结论 IL-5和IL-5Rα在ECRSw NP患者鼻息肉组织中表达量显著增加,可能与ECRSw NP发病及进展有关。

Objective To investigate the expressions and its significance of IL-5 and its receptor IL-5Rα in nasal polyp of patients suffering from chronic rhinosinusitis with nasal polyps（ CRSw NP）. Methods According to infiltrating degree of eosinophil in nasal polyps,89 patients with CRSw NP were divided into eosinophilic CRSw NP（ ECRSw NP） group（ n =41） and control group（ n = 48）. The expression of eosinophils in nasal polyps was observed by HE staining. The expression of IL-5 and IL-5Rα in nasal polyps was detected with RT-PCR technology. Results The proportions of allergic rhinitis,bilateral nasal polyps,number of involved sinus,severity of polyps and Lund-Mackay scores in ECRSw NP group were higher than those in the control group,the differences were statistically significant（ all P 〈0. 05）. The count and percentage of eosinophil,and Ig E level in ECRSw NP group were higher than those in the control group（ P 〈0. 05）. The average count of eosinophils per high power field in nasal polyps of ECRSw NP group was（ 36. 5 ± 14. 2）,which was higher than that of the control group,and the difference was also statistically significant（ P = 0. 000）. Pearson correlation analysis showed that the count of eosinophil in nasal polyps of ECRSw NP group was positively correlated with eosinophil count and percentage in peripheral blood（ r = 0. 703 and 0. 652,both P 〈0. 05）. The relative expression of IL-5 mRNA and IL-5Rα mRNA in nasal polyps of ECRSw NP group was higher than which of the control group,the differences were statistically significant（ P 〈0. 05）. Pearson correlation analysis showed that the relative expression of IL-5 mRNA in ECRSw NP group was positively correlated with that of IL-5Rα mRNA（ r = 0. 454,P = 0. 003）. Conclusion The expression of IL-5 and IL-5Rα in nasal polyps of patients with ECRSw NP is significantly increased,which may be related to the pathogenesis and progression of this disease.
