CD64与CD14在急性单核细胞相关的髓系白血病免疫分型鉴别诊断中的意义

Significance of CD64 and CD14 in immune typing and differential diagnosis of myeloid leukemia associated with acute monocytic

目的探讨CD64与CD14在急性单核细胞相关的髓系白血病免疫分型鉴别诊断中的意义,为临床诊治工作提供参考依据。方法选取2012年1月至2015年2月间收治的急性单核细胞相关的髓系白血病患者100例,采用流式细胞仪分析患者CD64、CD14抗原表达情况,对比分析检测结果。结果 CD64表达情况M0(急性粒细胞白血病微分化型)、M1(急性粒细胞白血病未成熟型)、M2(急性粒细胞白血病部分分化型)、M3(急性早幼粒细胞白血病)、M4(急性粒-单核细胞白血病)、M5(急性单核细胞白血病)阳性率分别为0.0%,0.0%,11.1%,47.4%,81.0%,100%。CD14表达情况M0、M1、M2、M3、M4、M5阳性率分别为0.0%,0.0%,0%,0%,52.4%,79.2%。45例M4/M5患者中,CD14 30例阳性,灵敏度为66.7%,特异性为100%,而CD64 41例阳性,灵敏度为93.3%,特异性为79.2%。结论 CD64诊断急性髓系白血病灵敏度显著高于CD14,而CD14诊断疾病的特异性明显高于CD64,各有优势、不足,二者相互结合应用能够明显改善检验准确度,减少误差,能够为临床诊断治疗等方面提供可靠的参考依据,值得借鉴。

Objective To investigate the significance of CD64 and CD14 in immune typing and differential diagnosis of myeloid leukemia associated with acute monocytic,provide a reference for clinical diagnosis and treatment. Methods 100 cases of myeloid leukemia associated with acute monocytic patients admitted between January 2012 to February 2015 were selected as the research object,CD64,CD14 antigen expression of patients was analyzed by flow cytometry,the test results were compared and analyzed. Results CD64 expression M0（ acute myeloid leukemia differential of the type）,M1（ immature acute myeloid leukemia）,M2（ acute myeloid leukemia part differentiation type）,M3（ acute promyelocytic leukemia）,M4（ acute myeloid- monocytic leukemia）,M5（ acute monocytic leukemia） positive rates were 0. 0%,0. 0%,11. 1%,47. 4%,81. 0% and 100%. Situation M0,M1,M2,M3,M4,M5 CD14 expression positive rate respectively were 0. 0%,0. 0%,0%,0%,52. 4%,79. 2%. 45 cases of M4 / M5 patients,CD14 30 cases were positive,the sensitivity was 66. 7%,specificity was100%,while CD64 41 cases were positive,the sensitivity was 93. 3%,and specificity was 79. 2%. Conclusion The sensitivity of CD64 diagnosis of acute myeloid leukemia was significantly higher than CD14,while specificity of CD14 diagnosis of the disease was significantly higher than CD64,have their own strengths,weaknesses,combined with each other both applications can significantly improve test accuracy,reduce errors,it is possible for the clinical provide diagnosis and treatment and other aspects of reliable reference,it is worth reference.
