复方甘草酸苷联合曲安奈德治疗口腔扁平苔藓的疗效观察

Clinical Efficacy Observation of Compound Glycyrrhizin Combined with Triamcinolone Acetonide in the Treatment of Oral Lichen Planus

目的:观察复方甘草酸苷联合曲安奈德治疗口腔扁平苔藓(OLP)的疗效及对患者免疫功能的影响。方法:100例患者随机分为观察组和对照组各50例,对照组口腔局部注射曲安奈德;观察组在对照组治疗基础上加服复方甘草酸苷片。4周为一疗程。采用视觉类比量表(VAS)和REU评分量表评价疼痛和OLP症状体征,比较两组临床疗效和细胞免疫功能,记录药品不良反应。结果:治疗后,两组VAS评分和REU评分均有显著下降,且观察组更低,差异均有统计学意义(P<0.05)。观察组总有效率为84.0%,明显高于对照组的66.0%(P<0.05)。观察组CD3^+及CD4^+T细胞亚群比例上升,CD19^+B细胞亚群比例下降,CD4^+/CD8^+降低;与对照组比较,差异有统计学意义(P<0.05)。两组药品不良反应轻微。结论:复方甘草酸苷联合曲安奈德对OLP对口腔扁平苔藓具有较好疗效,并可提高患者免疫功能。

Objective： To observe the impact of compound glycyrrhizin combined with triamcinolone acetonide on clinical effects and immune function in the patients with oral lichen planus（ OLP）. Methods： Totally 100 patients with OLP were randomly divided into the control group（50 cases） and the study group（50 cases）. The control group was locally injected with triamcinolone acetonide,and the study group was treated with compound glycyrrhizin tablets additionally. The treatment course was 4 weeks. Visual analogue scale（ VAS） and REU scoring system were used to assess pain and symptoms of OLP,the clinical effects and cellular immune function were compared,and the adverse reactions were recorded as well. Results： After the treatment,the VAS score and REU score respectively lower than those for the control group,and there were differents between the two groups（ P 〈0. 05）. The total effective rate was84. 0% in the study group,which was higher than that in the control group（66. 0%）（ P 〈0. 05）. The percentage of CD3＋ and CD4＋ cells was increased,that of CD19＋ cells and the ratio of CD4＋ / CD8＋ were decreased,and there were significant differences between the two groups（ P 〈0. 05）. The adverse reactions in both groups were mild. Conclusion： Compound glycyrrhizin combined with triamcinolone acetonide is effective for OLP and can improve the immune function in the patients.
