摘要
目的比较4种量表对于低位直肠癌保肛患者的肛门功能评价的一致性、可行性及影响肛门功能的因素。方法选取青岛大学附属医院黄岛院区普外科收治的直肠癌患者,行直肠癌根治术+预防性回肠造口术术后行回肠造口还纳的患者,通过Wexner量表、Vaizey量表、Pescatori量表、AMS量表对回肠造口还纳术1、3、6及12月后的患者的肛门功能进行评价。结果 1TNM分期Ⅰ期、Ⅱ期与Ⅲ期、Ⅳ期病人4种量表肛门功能评分差异均无统计学意义(P>0.05);2肿瘤位置越靠近肛缘,术后肛门功能恢复时间越长,肛门功能恢复情况越差;3大多数患者肛门控制稀便的能力于术后3月恢复,控制排气的能力在术后6月恢复;44种量表肛门功能评分间,两两相关性较好(P<0.05)。结论 Wexner量表、Vaizey量表、Pescatori量表、AMS量表、4种量表评估病人肛门功能具有较好的一致性和可行性。
Objective The feasibility and consistency of the evaluation of anal function by four kinds of scale for low rectal cancer patients after preserving anal operation and finding out factors affecting the anal function. Methods Selected rectal patients of general surgery in Affiliated Hospital of Qingdao University （Huangdao） and they have had radical resection of rectal cancer ＋ prophylactic colostomy and they have putted the ileum colostomy back after this surgery. Through Wexner scale,Vaizeyscale, Pescatori scale, AMS scale to evaluate the anal function at 1, 3, 6 and 12 months after operation. Results （3）TNM stage, the scores of those patients in I phrase, II phrase, III phrase, IV phrase evaluated by four scale had no statistical significance （P〈0.05）. （2）The anal function needs longer time to recover and the anal function recover worse when the tumor＇s location near the anus. （3）Most patients＂ anus control ability of loose stools has recovered at 3 months after surgery and the ability to control exhaust has recovered at 6 months after surgery. （4）4 kinds of scale have a good correlation （P〈0.05）. Conclusion Wexner scale, Vaizey scale, Pescatori scale,AMS scale have good consistency and feasibility in assessing the patients＇anal function.
