老年妇科肿瘤合并糖尿病围术期护理探讨

Perioperative Nursing for Gynecologic Tumor in the Elderly Complicated with Diabetes Mellitus

目的探讨老年妇科肿瘤患者合并糖尿病围手术时期护理方法及临床护理效果。方法选取2014年7月—2015年9月该院收治的88例老年妇科肿瘤合并糖尿病患者,按照随机数字分配法将其分为实验组和常规组,每组44例,实验组使用全方位定制式的围术期护理干预,常规组采用常规围术期护理干预,观察两组患者的手术时间、术中出血量,并对比两组患者的护理效果以及护理满意度。结果观察组中显效26例,有效15例,无效3例,护理总有效率为93.2%;常规组中显效16例,有效15例,无效13例,护理总有效率为70.5%。观察组患者的护理总有效率明显高于常规组(P<0.05)。此外观察组患者的手术时间以及术中出血量明显少于常规组(P<0.05)。观察组患者的护理满意度评分为(94.6±3.6)分,明显高于常规组的(81.5±2.3)分,三项指标组间差异有统计学意义。结论针对老年妇科肿瘤合并糖尿病患者实行全方位的围术期护理干预能够起到良好的护理效果,缩短手术时间,减少术中出血量,且患者满意度较高,值得广泛推广应用。

Objective To explore the method and clinical effect of perioperative nursing for gynecologic tumor in the elderly complicated with diabetes mellitus. Methods 88 elderly patients with gynecologic tumor and diabetes mellitus admitted in our hospital from July 2014 to September 2015 were selected and divided into the experimental group and conventional group with 44 patients in each according to the random number distribution method. Comprehensive and customized perioperative nursing intervention was given to the experimental group, and the conventional perioperative nursing intervention was given to the conventional group. The duration of procedure and intraoperative blood loss were observed in both groups. And the nursing efficacy and level of nursing satisfaction of the two groups were compared. Results In the observation group, the treatment was markedly effective in 26 cases, effective in 15 cases, ineffective in 3 cases, in the control group, the treatment was markedly effective in 16 cases, effective in 15 cases, ineffective in 13 cases, the overall nursing efficacy was much higher in the observation group than that in the control group（93.2% vs 70.5%）（P0.05）. The duration of procedure and the intraoperative blood loss was obviously less in the observation group than that in the control group, respectively（P 0.05）. The scores of level of nursing satisfaction were much higher in the observation group than those in the control group [（94.6 ± 3.6） points vs（81.5 ± 2.3） points] with statistically significant difference. Conclusion For gynecologic tumor in the elderly, comprehensive and customized perioperative nursing intervention can achieve good nursing effect, which can shorten the duration of procedure, reduce the intraoperative blood loss with high level of patient satisfaction, so it is worthy of wide application and promotion.
