腰椎间盘突出症保守治疗致马尾神经综合征28例临床分析 被引量:14

摘要 目的:研究腰椎间盘突出症保守治疗所导致的马尾神经综合征临床特点、手术方式及时机以评估其疗效。方法:收集完整的腰椎间盘突出症保守治疗所致的马尾神经28例,分析其起病情况、临床表现及分型、手术方式及时机等与疗效的关系。结果:所有患者术后临床症状明显缓解,各临床分型疗效差异无统计学意义;马尾神经完全性损伤患者预后比不完全性损伤患者差;急性患者24h内与24-48h手术疗效无明显差别,48h内与48h后手术疗效差异有统计学意义;融合器融合与植骨融合疗效差异无统计学意义;采用全椎板切除术疗效明显优于半椎板切除减压术及开窗髓核摘除术。结论:腰椎间盘突出症接受不适当的保守治疗后继发马尾神经综合征的患者进行早期减压手术,能彻底解除对马尾神经的压迫,缓解临床症状。
