声带麻痹患者气流动力学参数变化研究 被引量:1

摘要 目的对声带麻痹患者进行气流动力学检测,观察声门下压及相关参数的表现特征,探讨其在声带麻痹患者嗓音评估、诊断、冶疗、康复及预后判断中的价值。方法选择正常对照组30例,单侧声带麻痹患者58例(13例行患侧声带脂肪填充术),采用气流动力学测试仪对其进行测量。结果①各参数在不同性别间差异均不具有统计学意义(P〉O.05)。②/pa/音时声门下压最大,其次为/pi/音,最后为/ha/音。各音节间差异均具有统计学意义(P〈0.05)。③声带麻痹组声门下压、压力时程比与正常组相比差异均具有统计学意义(P〈0.05)。④声门下压及压力时程比在声带麻痹术后仍然高于正常(P〈0.05),但与术前相比都明显好转(P〈0.05)。结论所检测各参数中,声门下压、压力时程比在正常与声带麻痹组之间具有较好的区分度,可以作为临床声带麻痹诊断的客观检测指标。 Objective To observe and analyze the changes of airflow dynamic parameters in patients with vocal cord paralysis. Methods The airflow dynamic parameters of 58 patients with vocal cord paralysis and 30 normal controls, and the changes of airflow dynamic parameters before and after the surgery for vocal cord paralysis were observed. Results (1) There was no significant difference in all parameters between males and females (P〉0.05). (2)The subglottic pressure in/pa/ sound was the largest, followed by the/pi/, and finally for the/ba/. The differences were statistically significant (P〈0.05). (3)The differences between the vocal cord paralysis group and the normal control group in the subglottic pressure and the pressure/time ratio were statistically significant (P〈0.05). (4)The subglottic pressure and the pressure/time ratio were still higher than the control group after the surgery (P〈0.05), but the parameters were better as compared with the preoperative ones (P〈0.05). Conclusion Vocal cord paralysis may have different airflow dynamic parameters such as subglottic pressure and pressure/time ratio, which are valuable in the diagnosis of vocal cord lesions. These parameters can be used as objective indexes in clinical diagnosis of vocal cord paralysis and references for rehabilitation treatment.
