柱状缝扎术治疗环状混合痔36例临床观察

Clinical effect of columnar suture in treatment of circular mixed hemorrhoids: a report of 36 cases

目的:观察柱状缝扎术治疗环状混合痔的临床疗效。方法:将72例环状混合痔患者随机分为治疗组和对照组各36例。治疗组采用柱状缝扎术治疗,对照组采用外剥内扎术治疗。观察两组疗效、术后并发症(疼痛、出血、水肿)、术后肛门功能(肛门狭窄、肛门失禁)等情况,并进行比较分析。结果:总有效率治疗组为100%,对照组为97%,组间比较,差异无统计学意义(P>0.05);两组术后1、3、7、14d疼痛,术后7、14d出血,术后1、3d水肿及术后肛门狭窄评分比较,差异有统计学意义(P<0.05)。结论:柱状缝扎术治疗环状混合痔疗效肯定,并发症少,且安全性高,值得临床推广应用。

Objective： To observe the clinical effect of columnar suture in the treatment of circular mixed hemorrhoids. Methods： Seventy- two patients with circular mixed hemorrhoids were randomly and equally divided into treatment group and control group. The treatment group was treated with columnar suture,while the control group underwent Milligan- Morgan hemorrhoidectomy. The two groups were compared in terms of treatment outcome,postoperative complications（ pain,hemorrhage,and edema）,and postoperative anal function（ anal stenosis and anal incontinence）. Results： The treatment group had a nonsignificantly higher overall response rate than the control group（ 100% vs97%,P〉0. 05）. There were significant differences between the two groups in the scores of pain at 1,3,7,and 14 days after operation,bleeding at 7 and 14 days after operation,edema at 1 and 3 days after operation,and postoperative anal stenosis（ P〈0. 05）. Conclusion： Columnar suture has a good clinical effect in the treatment of circular mixed hemorrhoids,causes few complications,has high safety,and holds promise for clinical application.
