WANG Song-ling's experience in treating multiple system atrophy

王松龄教授临床善治神经科疑难病症,对多系统萎缩的治疗有独到见解。王师认为,本病属中医学"风痱病"范畴,多因先天禀赋不足,或后天旧病劳损,或年老体衰,或中气虚弱等而致肾元虚损;肝肾同源,肾元亏虚致肝血不足,肝木失养,疏泄失职,出现肝郁气滞、肝脾不和;本病进展缓慢,在疾病过程中可夹杂或风、或瘀、或痰等病理因素。本病临床证型多样,或肾阳不足,或肾阴亏损,或肾元不足,封藏失职,或肾气不足,脾气虚弱等;治疗应培补元气、温补肾阳或滋补肾阴治其本,祛风、化痰、祛瘀治其标,兼顾调摄肝、脾、心等脏腑,汤药为主,胶囊方为辅,多有良效。

Professor WANG Song-ling is good at treating the difficult miscellaneous diseases of neurology department and has formed insightful views in treating multiple system atrophy. Professor WANG attributes this disease to the category of hemiplegia after apoplexy, and the primary cause for this disease is deficiency of kidney qi which is usually caused by insufficient natural endowment, strain induced by acquired disease, elderly, insufficiency of qi in middle-jiao. Liver and kidney is homogeny, deficiency of kidney qi can lead to insufficient liver, and wood loss of nourishment and liver dysfunction might lead to stagnation of liver qi and incoordination between the liver and spleen eventually. This disease develops slowly with the accompanied pathological factors as wind, phlegm, blood stasis and so on. The clinical syndromes of this disease were various including syndrome of deficiency of kidney yang, syndrome of deficiency of kidney yin, syndrome of deficiency of kidney qi, and syndrome of deficiency of spleen qi. Clinical treatment for this disease should mainly focus on reinforcing the archaeus, warming and recuperating kidney yang or nourishing kidney yin to treat root cause of disease, and supplement with regulatingzang-fu viscera as liver, spleen and heart. The treatments for this disease gave priority to decoction, and supplemented with capsules, which had obtained good curative effect.
