温肾益肺汤对特发性水肿患者尿钠和尿钾的影响

Effect of Wenshen Yifei Tang on Urine Sodium and Urine Potassium of Patients with Idiopathic Edema

目的:观察温肾益肺汤对特发性水肿患者卧位和立位尿钠水平及尿钾水平的影响。方法:将60例特发性水肿患者随机分为2组,治疗组30例予温肾益肺汤治疗;对照组30例予氢氯噻嗪片治疗。2周为1疗程,2组均治疗2疗程,观察患者治疗前后卧位和立位尿钠水平及尿钾水平、症状积分、总有效率等。结果:2组治疗后卧位尿钠和立位尿钠均增加,与本组治疗前比较,差异均有统计学意义(P<0.01);治疗组治疗后立位尿钠水平高于对照组,差异有统计学意义(P<0.05);治疗组治疗后卧位尿钾和立位尿钾与本组治疗前比较,差异无统计学意义(P>0.05);对照组治疗后卧位尿钾和立位尿钾均增加,与本组治疗前比较,差异有统计学意义(P<0.01)。2组治疗后症状积分与本组治疗前比较,差异有统计学意义(P<0.01);治疗组治疗后症状积分与对照组比较,差异有统计学意义(P<0.05),治疗组优于对照组;治疗组随访3月后症状积分与对照组比较,差异有统计学意义(P<0.05)。2组总有效率比较,差异有统计学意义(P<0.05),治疗组优于对照组。结论:温肾益肺汤对特发性水肿患者有增加尿钠排泄的作用,对立位尿钠排泄作用明显,且不影响患者尿钾排泄,其近远期疗效均较理想。

Objective, To observe the effect of Wenshen Yifei tang on levels of urine sodium and urine potassium at lying and erect position in patients with diopathic edema. Method. Sixty cases of patients with diopathic edema were divided randomly into 2 groups. Thirty cases in the treatment group were given Wenshen Yifei tang, 30 cases in the control group were given hydrochlorothiazide tablets. The treatment covered 2 courses lasting 4 months. The levels of urine sodium and urine potassium at lying and erect position, symptomatic score and the total effective rate were observed before and after treatment. Result. Comparing with before treatment, the levels of urine sodium at lying and erect position in both groups were increased（P 〈 0.01）. The levels of urine sodium at erect position in the treatment group was much more higher than those in the control group（P〉 0.05）. Comparing with before treatment, the levels of urine potassium at lying and erect position in the treatment group were not change after treatment （P 〉 0.05）, while those in the control group were increased after treatment （P 〈 0.01）. In both groups, comparing the symptomatic scores before and after therapy, the differences were statistically significant（P 〈 0.01）. Those scores after treatment and three months after treatment in the treatment group were superior to those in the control group（P〈 0.05）. The total effective rate in the treatment group was superior to that in the control group. Conclusion. Wenshen Yifei tang can promote urine sodium excretion of patients with idiopathic edema, especially urine sodium excretion at erect position, and has no effect on urine potassium excretion, so it possesses ideal curative effects in the near and forward future.
