重力喂养联合抚触防治极低出生体重新生儿喂养不耐受的临床分析

Clinical analysis of gravity feeding combined with touch to prevent feeding intolerance of very low birth weight neonate

目的探讨重力喂养联合抚触防治极低出生体重新生儿喂养不耐受的临床效果。方法选择2012年1月1日至2013年6月30日本院收治的150例极低出生体重新生儿为研究对象，随机分为鼻饲组、重力组和联合组。鼻饲组：50例，采用常规间断鼻饲喂养；重力组：50例，采用常规重力喂养；联合组：50例，采用重力喂养，并给予抚触护理。对比分析三组患儿喂养不耐受发生情况、出生体重恢复时间、住院时间和完全肠内营养时间。结果鼻饲组喂养不耐受发生率明显高于重力组和联合组，差异有统计学意义（疋。=4．937，P=0．026；X2=6．828，P=0．009）；鼻饲组出生体重恢复时间、住院时间和完全肠内营养时间均明显长于比重力组，差异有统计学意义（t=5．211，P〈O．001；f=1．783，P=0．039；t=2．079，P=0．020）；重力组恢复出生体重时间和住院时间均明显长于联合组，差异有统计学意义（t=2．982，P=0．002；t=2．054，P=0．021）。结论重力喂养联合抚触可有效改善低出生体重儿喂养不耐受现状，缩短患儿出生体重恢复时间和住院时间。

Objective To explore the clinical effect of gravity feeding combined with touch to prevent feeding intolerance of very low birth weight neonate. Methods A total of 150 cases of very low birth weight neonate treated in our hospital from Jan. 1 , 2012 to Jun. 30 , 2013 were chosen as the objects, which were divided into nasal feeding group, gravity feeding group and combination group. 50 cases were treated with nasal feeding in nasal feeding group, an- other 50 cases were treated with gravity feeding in gravity feeding group, and the rest 50 cases were treated with gravity feeding combined with touch in combination group. The situation of feeding intolerance, time of birth weight recovery, length of stay and time of total enteral nutrition of the three groups were analyzed. Results The rate of feeding intoler- ance of nasal feeding group was much higher than that of gravity feeding group and combination group （X2 =4. 937, P= 0. 026 ; X2 = 6. 828, P = 0. 009） . The time of birth weight recovery, length of stay and time of total enteral nutrition of nasal feeding group were much longer than that of gravity feeding group （ t = 5.211, P〈0. 001 ; t = 1. 783, P = 0. 039 ; t = 2. 079, P = 0. 020） . The time of birth weight recovery and length of stay of gravity feeding group were much longer than that of combination group （t=2. 982, P=0. 002; t =2. 054, P=0. 021 ）, the differences were statistically signifi- cant. Conclusions Gravity feeding combined with touch can improve the situation of feeding intolerance of very low birth weight neonate, shorten the time of birth weizht recovery and lenzth of stay.
