急性脑梗死患者糖化血红蛋白与颈动脉斑块厚度的相关性研究

Relationship between Hb A_1c and the thickness of carotid atherosclerosis plaque in acute cerebral infarction patients

目的探讨急性脑梗死患者糖化血红蛋白(Hb A1c)与颈动脉粥样硬化斑块厚度的相关性。方法选择住院的急性脑梗死患者200例,依据有无合并2型糖尿病将其分为糖尿病组(T2DM组)100例,非糖尿病组(NDM组)100例。所有患者均检测Hb A1c、血脂、同型半胱氨酸(Hcy),并应用彩色多普勒超声仪测定颈动脉斑块的有无,测定其最大斑块厚度(MTCAP)。探讨其相关性及危险因素。结果 1T2DM组和NDM组MTCAP、收缩压(SBP)、Hb A1c、血清总胆固醇(TC)比较差异有统计学意义(P<0.05)。2相关性分析:T2DM组中Hb A1c、TC、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、Hcy与MTCAP呈正相关(r=0.243,0.235,0.395,0.278,0.215,P<0.05),以MTCAP为因变量,以Hb A1c、TC、TG、LDL-C、Hcy为自变量进行多元线性回归分析,TC、TG、Hcy 3个变量进入最终方程。结论急性脑梗死合并2型糖尿病患者的Hb A1c与MTCAP呈正相关,但并不是MTCAP的主要危险因素。

Objective To explore the relationship between Hb A_1c and the thickness of carotid atherosclerosis plaque in Chinese acute cerebral infarction patients. Methods A total of 200 patients with acute cerebral infarction were divided into T2 DM complicated with T_2DM（group A,n = 100）and NDM without T_2DM（ group B,n = 100）.The levels of blood fat,glycated hemoglobin（Hb A1c）and serum homocysteine（Hcy）were detected. All patients were received carotid ultrasonography screening,and the maximal thickness of carotid atherosclerosis plaque（MTCAP）were assessed. Results The differences in mean MTCAP,Hb Acl,systemic blood pressure（SBP）and total cholesterol（TC）between group A and B were statistically significant（P〈0.05）.The correlation analysis showed that MTCAP was positively correlated with Hb A1 c,TC,triglyceride（TG）,low densitylipoprotein-cholesterol（LDL-C）and Hcy（r=0.243,0.235,0.395,0.278,0.215,P〈0.05）.The multiple linear stepwise regression analysis showed that the TC,TG and Hcy went into the final equation with MTCAP as the dependent variables.Conclusion For Chinese acute infarction patients,Hb A_1c positively correlates to MTCAP.But Hb A_1c is not an independent risk factor to the presence of carotid atherosclerosis plaque.
