腹腔镜辅助胃癌根治术后并发症的Clavien-Dindo分级及危险因素分析 被引量:32

Analysis of risk factors and Clavien-Dindo classification of complications after laparoscopy-assisted gastrectomy

摘要 目的分析腹腔镜辅助胃癌根治术后并发症的Clavien—Dindo分级,探讨影响术后并发症发生的危险因素及其与并发症Clavien—Dindo分级的关系。方法采用回顾性病例对照研究方法。收集2004年1月至2014年12月上海交通大学医学院附属瑞金医院收治的578例行腹腔镜辅助胃癌根治术患者的临床病理资料。观察指标包括:(1)术后并发症情况,采用Clavien—Dindo分级。(2)单因素和多因素分析指标:患者基本情况,手术相关因素,肿瘤相关因素。(3)分析影响行腹腔镜辅助胃癌根治术患者发生术后并发症的独立危险因素与术后并发症Clavien.Dindo分级的关系。采用门诊和电话方式进行随访,随访患者术后生存及肿瘤复发转移情况,随访时间截至2015年7月。正态分布的计量资料以元±s表示。单因素分析和计数资料的比较采用χ^2检验,多因素分析采用Logistic回归。结果578例患者中,82例发生术后并发症:32例吻合口瘘,10例术后肠梗阻,9例腹腔内出血,9例腹腔感染,8例吻合口出血,8例术后胃排空障碍,6例吻合口狭窄,6例十二指肠残端瘘,6例淋巴瘘,2例肠坏死,2例切口感染。部分患者同时合并多项并发症。术后并发症Clavien—Dindo分级情况:Ⅰ级2例,Ⅱ级60例,Ⅲa级6例,Ⅲb级12例,Ⅳ级1例,Ⅴ级1例。单因素分析结果显示:患者BMI、吻合方式、术中出血量、肿瘤直径、肿瘤N分期是影响腹腔镜辅助胃癌根治术后并发症发生的相关因素(χ^2=12.672,15.608,3.948,3.989,17.214,P〈0.05)。多因素分析结果显示:BMI≥24kg/m^2、BillrothⅡ式吻合术是影响腹腔镜辅助胃癌根治术患者发生术后并发症的独立危险因素(RR=2.363,3.581,95%CI:1.421~3.929,1.680~7.632,P〈0.05)。术后并发症Clavien—Dindo Ⅰ级、Ⅱ级、Ⅲa级、Ⅲb级、Ⅳ级、Ⅴ级在BMI〈24kg/m^2的患者例数分别为1、33、6、8、1、0例,在BMI≥24kg/m^2的患者例数分别为1、27、0、4、0、1例;行BillrothⅠ式吻合术的患者例数分别为2、24、2、5、1、1例,行BillrothⅡ式吻合术的患者例数分别为0、11、0、3、0、0例,行Roux—en—Y吻合术的患者例数分别为0、25、3、4、0、0例,行食管胃吻合术的患者例数分别为0、0、1、0、0、0例。不同BMI和吻合方式的患者,腹腔镜辅助胃癌根治术后并发症Clavien—Dindo分级比较,差异均有统计学意义(χ^2=20.237,36.104,P〈0.05)。578例患者中,1例于术后1个月死亡,534例获得术后随访。随访时间为7~136个月,中位随访时间为44个月。随访期间,49例患者死亡(其中31例为肿瘤相关性、18例为非肿瘤相关性),39例出现肿瘤复发和(或)转移。其余患者均无瘤生存。结论腹腔镜辅助胃癌根治术后并发症以吻合口瘘较多见,Clavien—Dindo分级多为Ⅱ级。BMI≥24ks/m^2、BillrothⅡ式吻合术是影响行腹腔镜辅助胃癌根治术患者发生术后并发症的独立危险因素。患者BMI和吻合方式影响术后并发症Clavien-Dindo分级。 Objective To analyze the Clavien-Dindo classification of complications after laparoscopy- assisted gastrectomy (LAG) and investigate the relationship between risk factors and Clavien-Dindo classification of complications.
