睾丸表皮样囊肿24例诊断治疗分析

Analysis of diagnosis and treatment of testicular epidermoid cyst in 24 cases

目的：分析睾丸表皮样囊肿的临床特点,进一步提高对睾丸表皮样囊肿的诊治水平,降低临床误诊率。方法：回顾性分析我院2000年3月~2015年1月收治的24例术后病理诊断为睾丸表皮样囊肿的诊治过程。结果：22例行保留睾丸的肿瘤剜除术,2例行睾丸根治性切除术。术中20例行快速冷冻病理检查,均诊断为睾丸表皮样囊肿。术后随访4个月~14年,无肿瘤的复发及转移。结论：睾丸表皮样囊肿是临床上比较少见的睾丸良性肿瘤,预后较好,易被误诊为睾丸恶性肿瘤。病史、超声及实验室检查有助于术前诊断,术中快速冷冻病理对手术方式的选择有决定性作用。目前治疗主张行保留睾丸的肿瘤剜除术。

Objective： To analyse the clinical characteristics of testicular epidermoid cyst, improve the diagnosis and treatment, and reduce clinical misdiagnosis rate. Method： From March 2000 to January 2015, 24 cases of testicular epidermoid cyst diagnosed by postoperative pathological examination were treated in our hospital. The epidemiology, clinical manifestations, features of image examination, features of pathology and treatment were an- alysed retrospectively. Result： Twenty-two cases underwent tumor enucleation with testis preserving, and the oth- er two cases received testicular radical resection. Intraoperative frozen section were performed in 20 cases and all of them were confirmed the diagnosis of testieular epidermoid cyst. No local recurrence or peripheral metastasis was found during the follow-up period from 4 months to 14 years. Conclusion： Testicular epidermoid cyst is a kind of rare benign testicular tumor, so it＇s prone to be misdiagnosed as testicular malignant tumor. The prognosis is good. Medical history, ultrasonic examination and laboratory examination are important for preoperative diagnosis, and intraoperative frozen section is crucial to the choice of the surgical method. Nowadays tumor enucleation with testis preserving is recommended.
