全胸腔镜非小细胞肺癌根治术临床研究

A retrospective study on the treatment of non small cell lung cancer via totally video assisted thoracoscopy

目的探讨全胸腔镜下非小细胞肺癌根治术中区域淋巴结清扫的可行性、安全性及临床价值。方法选取我院2014年1月至2015年12月收治的60例周围型非小细胞肺癌患者,分别施行全胸腔镜(n=32)及传统开胸(n=28)肺癌根治术。比较两组手术时间、术中出血量、术后住院时间、术中淋巴结清扫数目、术后阳性淋巴结数等情况。结果与传统开胸手术组比较,全胸腔镜下肺癌根治术组术中出血量、术后住院时间减少,差异有统计学意义(P<0.05);两组手术时间、淋巴结清扫数目、阳性淋巴结数目方面比较,差异无统计学意义(P>0.05)。结论全胸腔镜下肺癌根治术淋巴结清扫,在减少创伤的同时,对周围型肺癌是一种安全、有效、创伤小并能够达到根治目的的手术选择。

Objective To explore the feasibility,safety and clinical value of totally video assisted thoracoscopy on the treatment of non-small cell lung cancer for regional lymph node cleaning. Methods A retrospective study was performed on 60 cases of patients with non small cell lung cancer received from January 2014 to December 2014. According to the surgical approaches,all 60 cases were divided into two groups：the thoracoscopy group（ n = 32） and traditional thoracotomy group（ n = 28）. Clinical features such as operating time,intraoperative blood loss,postoperative hospital stay,number of intraoperative lymph node cleaning,and number of positive lymph nodes after surgery were compared between the two groups. Results When compared to the traditional thoracotomy group,both intraoperative blood loss and postoperative hospitalization time reduced in patients of the thoracotomy group（ P〈 0. 05）. While,there were no significant difference between the two groups when comparison of the lymph node surgery time,number,and number of positive lymph nodes（ P〉 0. 05）. Conclusion It is a safe and effective approach to perform lymphadenectomy in radical resection of pulmonary carcinoma for the treatment of non small cell lung cancer,which could reduce surgical trauma and achieve a radical cure.
