住院药房针剂调配差错分析与防范

Analysis and Prevention of Injections Dispensing Errors in Inpatient Pharmacy

目的:探索减少住院药房针剂调配差错的措施,提高药品调配质量。方法:对我院2013年和2014年住院药房《针剂调配差错登记本》登记的内部差错进行回顾性分析。结果:2013年共发生内部调配差错822例,其中药物数量差错275例,药物品种差错224例,漏摆181例,药品规格差错92例,生产厂家错误50例;2014年共发生内部调配差错480例,其中药物品种差错162例,数量差错159例,漏摆99例,药品规格差错35例,生产厂家错误25例。2014年比2013年共减少调配差错342例。结论:通过建立住院药房调配标准化体系和调配工作各个环节的标准操作规程,提高药学人员业务素质及工作责任心,可有效防范处方调配差错的发生。

Objective： To explore practical measures of reducing dispensing errors in the pharmacies, and improve the quality of drug dispensing. Methods： The dispensing errors recorded in the Drug Deployment Errors Registration in inpatient pharmacy from 2013 to 2014, were collected and analyzed retrospectively. Results： There were 822 dispensing errors in total in 2013, among which 275 were cases of number errors, 224 were cases of species errors, 181 were cases of omissions errors, 92 were cases of drug specifications errors and 50 were cases of manufacturer errors. And there were 480 dispensing errors in total in 2014, among which 162 were cases of species errors, 159 were cases of number errors, 99 were cases of omissions errors, 35 were cases of drug specifications errors and 25 were cases of manufacturer errors. In 2014, cases of dispensing errors were reduced by 342 as compared with 2013. Conclusion： The occurrence of drug dispensing errors is effectively prevented by establishing the dispensing standardized system in inpatient pharmacy, allocating the standard operating procedures of the various links in the drug dispensing work, improving the professional quality as well as work responsibility of pharmacy staffs.
