舒血宁注射液临床用药剂量与肾功能及肝功能的相关性研究

The Effect of the clinical dosage of Shuxuening injection on patients with liver function and kidney function

目的探讨舒血宁注射液临床用药剂量与肾功能及肝功能的相关性。方法回顾性分析我院信息系统数据库中临床使用舒血宁注射液治疗的患者病历资料,按照舒血宁注射液使用剂量将患者分为观察组和对照组,对照组按照说明书推荐剂量使用舒血宁注射液,观察组超过说明书推荐剂量使用舒血宁注射液,采用倾斜性评分法(GBM)对肝肾功能的影响因素进行平衡,比较两组肝功能指标及肾功能指标变化。结果经典logistic回归分析显示舒血宁注射液对患者血肌酐(Scr)和谷丙转氨酶(ALT)可能有影响(P<0.05),但倾斜性评分后显示舒血宁注射液对Scr和ALT无显著影响(P>0.05)。经典logistic回归分析和倾斜性评分显示舒血宁注射液对血尿素氮(BUN)有显著影响(P<0.05)。经典logistic回归分析和倾斜性评分显示舒血宁注射液对谷草转氨酶(AST)均无显著影响(P>0.05)。结论本文属于回顾性研究方法,对于临床中大量混杂因素仍存在潜在的偏差,舒血宁注射液对患者肝功能和肾功能是否存在不良影响,仍需进一步进行研究,本文研究结果仅能作为临床用药参考。

Objective To explore the effect of Shuxuening injection dosage on patients with liver function and kidney function. Methods Retrospective analysis of the medical records of patients with the use of Shuxuening injection in our hospital, the subjects were divided into the observation group and the control group according to the dose of Shuxuening Injection, including 826 patients who received treatment with more than the recommended dose（ the observation group） and 1349 patients who received treatment with recommended dose（ the control group）. The influence factors of liver and kidney function were balanced by using the oblique scoring method, two groups of Liver and kidney function indexes were compared. Results The classical logistic regression analysis showed Scr and ALT may be affected with Shuxuening injection （P 〈0. 05）, however, the GBM score showed Shuxuening injection had no significant effect on Scr and ALT （P 〉 0. 05）. The classical logistic regression analysis and the GBM score showed Shuxuening injection has a significant influence on BUN（P 〈0. 05）. The classical logistic regression analysis and the GBM score showed Shuxuening injection had no significant effect on AST （P 〉 0. 05）. Conclusion There are potential deviations in a large number of confounding factors in the course of clinical treatment, the effect of Shuxuening Injection on liver function and renal function need further study, the results can only be used as a reference for clinical drug use.
