基于互联网的移动远程胎心监护的实现和一致性评价 被引量:18

摘要 目的探讨基于互联网的移动远程胎心监护的实现及与传统胎心监护结果的一致性。方法利用多普勒超声探头采集胎心信号,通过蓝牙通讯技术将监测信号传输至智能手机APP客户端,数据通过互联网通讯技术与胎心监护中心互联以实现远程胎心监护,对476例孕晚期孕妇随机进行远程和传统胎心监护,自身前后配对,采用Bland-Altman LoA方法进行两种监护方法的一致性评价,并采用MOVER方法对95%LoA进行可信区间估计。结果远程和传统胎心监护得分的均差为-0.09,两种监护方法 95%一致限LoA为(-0.72,0.54),按MOVER法计算的95%一致限LoA可信区间为(-0.78,0.60)。结论基于互联网的移动远程胎心监护实现可行,与传统胎心监测结果一致,是孕期常规保健的有效补充。 Objective To explore the implementation of moved remote fatal monitoring system based on internet and the agreement evaluation with traditional fetal heart rate monitoring. Methods The fetal signals was collected by Doppler ultrasound probe,which monitor signals was transmitted to the smart phone APP client by Bluetooth communication technology,remote fetal heart rate monitoring was implemented by the interconnected of the fetal heart rate monitoring center and the Internet communication technology.476 later pregnancy women were carried out remote and traditional fetal heart rate monitoring a randomly,which matched themselves before and after. Bland-Altman LoA method was used to evaluate the agreement of the two monitoring methods,while MOVER method was used to estimate 95% confidence interval for LoA. Results The mean difference of score between remote and traditional fetal heart rate monitoring was- 0. 09. The agreement of 95% LoA of the two monitoring methods was(- 0. 72,0. 54),which the 95%confidence interval agreement of LoA was(- 0. 78,0. 60) by MOVER method. Conclusion The implementation of Moved Remote Fetal Heart Rate Monitoring based on internet is visible,and the results are agreement with the traditional fetal heart monitoring,this is an effective complement for conventional prenatal care.
