新疆维吾尔自治区新生儿复苏项目开展及培训状况调查分析

Analysis on the program management and training effect of neonatal resuscitation program in Xinjiang

目的了解新生儿复苏项目实施10年来新疆的项目管理及培训情况。方法采用分层整群抽样方法,按行政区域将新疆划分为南疆地区、北疆地区,在南北疆随机选取1个地(州),对抽中地(州)市级卫生行政部门、116家医疗机构、956名产科、儿科、麻醉科医护人员及41名各级师资开展一系列问卷调查、访谈、座谈。结果 1项目管理:到第二周期为止,南、北疆地区都建立了新生儿复苏领导小组、制定了督导计划并深入开展乡级培训。2院内培训制度:县级及县级以上医疗机构培训执行较好,乡级较差。南疆地区由于儿科医生缺乏,相关培训制度未建立。3人员培训:82%的医护人员认为复苏技术中最难掌握的是气管插管;40%的医护人员认为自己缺乏在复苏现场评估新生儿状态并决定下一步操作的判断能力。结论新疆新生儿复苏工作要得到各级卫生行政部门的重视。未来应加大经费支持力度,做好各级培训、督导、考核,以巩固培训效果。

Objective To understand the neonatal resuscitation project（ NRP） development and training situation in Xinjiang during the ten years. Methods Using stratified cluster sampling method,a state of Southern Xinjiang was selected randomly,and also in Northern Xinjiang. The municipal public health administrative department,116 medical institutions,956 medical staffs and 41 teachers at all levels were all carried out a series of questionnaire survey,interviews,discussion. Results 1 Program management：By the end of the second phase of the NRP,the public health administrative department of the Northern and Southern Xinjiang have both established neonatal resuscitation group,developed the monitoring and evaluation plan and carried out the trainings at township level. 2In-hospital training regulation： the in-hospital training regulations executed better in hospitals of county and above level than that in village level. In hospitals of Southern Xinjiang,recommended regulations were not set up due to limiting of pediatricians.3Health care providers Training： 82% of the health care providers believed that the most difficult part of NRP skill was endotracheal intubation; 40% of the health care providers self-reported that they had lack confidence in evaluating newborn condition and determining the next step of resuscitation. Conclusion The NRP in Xinjiang should be received more support from the governments and health administrative departments. Future inputs in financial support should be emphasized in order to strengthen ability in training,monitoring and supervision to consolidate the training effect.
