感染性休克早期心肌损伤患者应用β受体阻滞剂的安全性探讨 被引量:12

摘要 目的:探讨感染性休克早期心肌损伤患者应用β受体阻滞剂(美托洛尔)的安全性。方法:回顾性分析近3年我院收治感染性休克早期合并心肌损伤42例患者的临床资料,根据早期是否应用美托洛尔分为观察组(22例)和对照组(20例)。对照组:按照2012年国际脓毒症指南进行了规范化治疗;观察组:在规范化治疗的基础上,早期通过静脉注射美托洛尔使心率下降20%左右,持续维持6 h。观察记录2组患者心功能状态、血流动力学、入选后24 h复苏液体量及血管活性药物剂量以及心律失常情况。结果:①2组患者给药前心功能状态、血流动力学、氧代谢参数比较,差异均无统计学意义(均P>0.05)。②2组患者入选后24 h液体复苏量和血管活性药物剂量比较,差异无统计学意义(P>0.05)。③与治疗前比较,观察组给药后心率和心脏指数分别为(96±18)次/min、(3.3±1.6)L/(min·m^2),比给药前(126±20)次/min、(3.9±2.4)L/(min·m^2)明显降低(P<0.05)。④观察组给药前、后血流动力学参数(平均动脉压、中心静脉压、肺动脉嵌顿压、肺循环阻力指数、体循环阻力指数、每搏量指数等)和心律失常发生率均无显著变化(P>0.05)。⑤观察组给药后血乳酸水平(9.4±3.5)mmol/L比给药前(13.5±4.3)mmol/L明显降低(P<0.05);而2组治疗前、后混合静脉血氧饱和度比较差异无统计学意义(均P>0.05)。结论:美托洛尔能够安全、有效地降低感染性休克早期心肌损伤患者的心率和心输出量,但对循环功能和全身组织灌注影响小,不增加心律失常发生率。
