输尿管镜扩张法和钬激光内切开法治疗输尿管狭窄疗效观察

Clinical observation of ureteroscopic expansion method and holmium laser incision method in the treatment of ureterostenosis

目的探讨输尿管镜扩张法和钬激光内切开法治疗输尿管狭窄的临床疗效。方法将64例输尿管狭窄患者按随机数字表法分为两组各32例,对照组给予冷刀内切开法治疗,试验组给予输尿管镜扩张法和钬激光内切开法治疗,比较两组患者治疗前后输尿管狭窄及其近端扩张程度、肾集合系统分离程度与最大尿流量、相关临床指标与并发症。结果治疗后试验组患者输尿管狭窄段狭窄程度、最大尿流量明显高于对照组,狭窄近端输尿管扩张程度、肾集合系统分离程度、尿道扩张时间及再手术率明显低于对照组(P<0.05);两组总并发症发生率差异无统计学意义(P>0.05)。结论输尿管镜扩张法和钬激光内切开法治疗输尿管狭窄的临床疗效显著。

Objective To investigate the clinical effectiveness of ureteroscopic expansion method and holmium laser incision method in the treatment of ureterostenosis.Methods Sixty-four patients with ureterostenosis were randomly divided into control and experimental groups,32 in each group. The control group received the treatment of cold knife incision while the experimental group received the treatment of ureteroscopic expansion method and holmium laser incision method.The proximal urethral dilation,degree of separation of renal collecting system and maximum urinary flow rate,relative clinical indicators and complications were compared before and after treatment between the two groups.Results After the treatments,the proximal urethral dilation,degree of separation of renal collecting system,maximum urinary flow rate,urethral expansion time and re-operation rate in the experimental group were all better than that in the control group（ all P 〈0. 05）.However,there was no significant difference in the rate of complications between the two groups（ P 〉0. 05）.Conclusion The clinical curative effect of the two methods is distinct.
