碱性成纤维细胞生长因子对大鼠肩袖撕裂重建术后早期腱-骨界面愈合的影响 被引量:3

摘要 目的观察碱性成纤维细胞生长因子(BFGF)对大鼠肩袖撕裂重建术后早期腱-骨界面的修复效果。方法 45只Wistar大鼠随机分为高剂量组、低剂量组和空白组,每组15只。取双侧肩袖,大结节止点处横断撕裂冈上肌腱全层,重建肩袖止点原位缝合(双排固定法),高、低剂量组在腱-骨界面分别一次性注射以纤维蛋白胶为载体的BFGF缓释体50、10μg,空白组除手术外不给予任何干预措施。术后1、2、3周处死动物,取冈上肌腱为标本进行免疫组化和生物力学分析。结果镜下见高剂量组术后3周时Sharpey's纤维显著增多,胶原纤维长入腱-骨界面。3组腱-骨界面最大抗拉强度和冈上肌腱术侧/健侧最大载荷百分比随时间延长呈升高趋势(P<0.05)。高剂量组术后各时间点腱-骨界面最大抗拉强度、冈上肌腱术侧/健侧最大载荷百分比均显著高于低剂量组及空白组(P<0.05),术后1、3周腱-骨界面最大横断面面积均显著大于低剂量组及空白组(P<0.05),术后2周腱-骨界面刚度均高于低剂量组及空白组(P<0.05)。低剂量组术后各时间点腱-骨界面最大抗拉强度、冈上肌腱术侧/健侧最大载荷百分比均显著高于空白组(P<0.05),术后3周腱-骨界面最大横断面面积显著大于空白组(P<0.05),术后2周腱-骨界面刚度显著高于空白组(P<0.05)。结论高剂量BFGF可诱导断面再生成直接止点的复杂结构,促进肩袖损伤的愈合,在肩袖损伤修复早期效果较佳,而低剂量BFGF可以起到肌腱修复的作用,在临床有一定的应用价值。
