中老年膝骨关节炎发病病因及相关危险因素分析

Cause and risk factors analysis on knee osteoarthritis of middle aged and old reople

目的探讨影响中老年膝骨关节炎(KOA)发病病因及其相关危险因素,为早期防治KOA提供依据。方法采用病例对照研究法,将确诊的125例KOA病例作为病例组,同时收集125名健康体检人群作为对照组,对有所有研究对象进行问卷调查来收集KOA发病的相关因素;采用单因素、多因素Logistic回归法来分析KOA发病的相关危险因素。结果多因素Logistic回归分析结果显示年龄(OR=3.438)、肥胖(OR=1.852)、常居潮湿阴暗环境(OR=1.982)、不锻炼(OR=3.680)、重体力劳动职业(OR=3.445)、骨关节炎家族史(OR=6.746)、营养不良(OR=2.568)均为KOA发病的危险因素。结论 KOA发病的独立危险因素包括高龄、肥胖、常居潮湿阴暗环境、缺乏体育锻炼、从事重体力劳动职业、有骨关节炎家族史及长期营养不良,应针对这些可改变的独立危险因素制订干预措施。

Objective To explore cause and correlated risk factors of the incidence of knee osteoarthritis（KOA） of middle aged and old people,and to provide basis for early preventing and intervening.Methods With case-control study method,125 definite cases of KOA patients were collected as case group,and 125 cases of healthy examination people were collected as control group.All of these study subjects were investigated by questionnaire.And we collected correlated factors of KOA.Single factor and multiple factors logistic regression analysis methods were used to screen risk factors of KOA.Results Multiple factors analysis result showed that the risk factors of KOA included age（OR =3.438）,obesity（OR =1.852）,usually dwelling in a humid and dark environment（OR =1.982）,lack of exercise（OR =3.680）,heavy manual labor occupation（OR =3.445）,family history of osteoarthritis（OR =6.746）and malnutrition（OR =2.568）.Conclusion The independent risk factors of KOA included high age,obesity,usually dwelling in a humid and dark environment,lack of exercise,heavy manual labor occupation,family history of osteoarthritis and malnutrition.So we should formulate intervention measures aiming directly at these independent risk factors which could be changed.
