临床药师干预医院门诊中成药注射剂应用效果分析

Influence of Clinical Pharmacists' Intervention on Chinese Patent Medicine Injection

目的分析临床药师干预前后中成药注射剂的应用情况,保证临床用药安全。方法抽取干预后第2-4个月的处方（3 021张）作为研究组,调取干预前（2014年1月至3月）的2 800张处方作为对照组,比较两组处方的应用水平。结果临床药师干预后不规范率为4.79%,较干预前的32.70%显著降低（χ^28.12,P〈0.05）;应用中存在的主要问题为药品用量、溶剂用量与溶剂选择不当。干预前,药品用量不当3张、溶剂用量不当35张、溶剂选择不当14张,而干预后存在问题的处方数分别为1,6,2张,均明显少于干预前（χ^24.98,10.02,6.12,P〈0.05）;干预前不规范中成药注射剂处方涉及8种药物,干预后仅3种,且对丹红注射液的不规范使用率明显下降（χ^23.21,P〈0.05）。结论临床药师在医院门诊的中成药注射剂合理用药中起着关键性作用,干预后药物的规范性明显提高。

Objective To explore the application of Chinese patent medicine injection before and after the clinical pharmacist intervention /to ensure the safety of clinical medicine.Methods 3 021 pieces of prescription after 2-4 months after intervention were selected as the study group,and 2 800 pieces of prescription from January to March 2014 before the intervention were selected as the control group.The levels of prescription application of the two groups were compared.Results After clinical pharmacist intervention,the rate of non-standard application was 4.79%,which was significantly lower than 32.70%before the intervention（χ^28.12,P〈0.05）;the main problems existing in the application of Chinese patent medicine injection were selection of solvent,solvent dosage discomfort and drug dosage discomfort.Before the intervention /Selection of solvent discomfort was 14 pieces,the amount of solvent discomfort was 35 pieces,drug dosage discomfort was 3 pieces,while after clinical pharmacists＇ intervention,the numbers were 1,6,2 pieces respectively,which was significantly less than before intervention（χ^24.98,10.02,6.12,P〈0.05）;the Chinese patent medicine injections that were in the non-standard prescnptions involved 8 drugs before intervention,while after clinical pharmacist intervention,the number was only 3,and the non-standard usage rate of Danhong Injection decreased obviously（χ^23.21,P〈0.05）.Conclusion Clinical pharmacists play a key role in the rational drug use of the hospita＇s outpatient Chinese patent medicine injections,and the standard rate of drugs significantly improved after the intervention.
