摘要 目的:了解骨关节炎(OA)和骨质疏松症(OP)患者体重指数与骨密度的相关性,分析椎体压缩性骨折发生率及相关危险因素。方法:OA 464例,OP 404例采用双能X线(DEXA)测定患者腰椎的骨密度(BMD),依体重指数(BMI)分组:BMI≥25kg/m^2超重组(A),20kg/m^2≤BMI<25kg/m^2体重正常组(B),BMI<20kg/m^2低体重组(C)。OP进行同位素骨扫描(ECT)了解患者椎体压缩性骨折及各组间相关指标比较。结果:两类患者BMD为低体重组最低。且OA伴低体重者更易发生OP。OP患者行ECT,发生椎体压缩性骨折35%,且多个椎体压缩性骨折26%。体重指数A组,年龄>70更易发生OA及OP,同时伴发椎体压缩性骨折的风险高。结论:OA低BMI者更易发生OP。两病共患者发生椎体压缩性骨折高,年龄大于70岁BMIA组易发生椎体压缩性骨折。 Objective: To investigate the correlation between body mass index and bone density in patients with osteoarthritis( OS) and osteoporosis( OP),to analyze the incidence and associated of vertebral compression fractures risk factors. Method: 464 cases of OA an 404 cases of OP were selected as the objects.The bone mineral density( BMD) of lunbar vertebra was measured. by dual energy X-ray( DEXA). According to body mass index( BMI) the patients were divided into three groups: BMI overweight group ≥ 25kg/ m-2,normal weight group 20 kg / m-2≤BMI 25 kg / m-2,low weight group BMI 20 kg / m-2. The patients with OP adopted oprectived isotope bone scanning( ECT) to learn the comparisons between the relevant indicators in three groups and had vertebral compression fractures. Results: Patients in low weight group had the lowest BMD. And the patients with OA with low birth weight were more likely to occur OP. The patients with OP underwent ECT,and the occurrence of vertebral compression fractures was by 35%,and the rate of multiple vertebral compression fractures was 26%. In BMI A group the patients whose ages were more than 70 were more likely to suffer OA and OP,with the high risk of vertebral compression fractures. Conclusion: The patients with OA and low BMI were more likely to suffer OP. Those patients with OA and OP had high risk of vertebral compression fractures. The patients in BMIA group whose ages were more than 70 prone to suffer vertebral compression fractures.
