利培酮联合高压氧治疗阴性症状为主的精神分裂症患者的效果观察

Efficacy of risperidone combined with hyperbaric oxygen therapy in the treatment of schizophrenia with negative symptoms

目的 探讨利培酮联合高压氧治疗以阴性症状为主的精神分裂症患者的临床疗效。方法 选取2012年4月~2013年12月在我院治疗的105例以阴性症状为主的精神分裂症患者,将其随机分为治疗组、高压氧组、研究组,各35例。治疗组予利培酮治疗;高压氧组予高压氧治疗;研究组予利培酮联合高压氧治疗。观察3组入院时、第4周、第8周、第12周末时阳性症状和阴性症状量表（PANSS）及不良反应和病程,以及随访1年复发率及医疗总费用等。结果 治疗组总有效率为45.7%,高压氧组总有效率为40.0%,研究组总有效率为77.1%,3组总有效率比较差异有统计学意义（P〈0.05）。研究组病程、医疗总费用、复发率均优于其余两组（P〈0.05）。研究组在第4周末、第8周末、第12周末时TESS总分均低于其余两组（P〈0.01）;不良反应均以椎体外系症状为主;在程度及发生率上由高到低依次为治疗组、研究组、高压氧组,其中治疗组、研究组不良反应发生人数为依次为19例（54.3%）、9例（25.7%）。研究组在第12周末时PANSS总分、阴性因子分及PANSS各阴性因子评分均明显低于同组治疗前,且低于同期其余两组,差异有统计学意义（P〈0.05）。结论 高压氧治疗以阴性症状为主的精神分裂症患者,安全有效;与利培酮联合应用,能有效提高临床疗效,减少不良反应,缩短病程,降低复发率,减少医疗费用等。

Objective To investigate the clinical effect of risperidone combined with hyperbaric oxygen therapy in the treatment of schizophrenia with negative symptoms. Methods 105 patients with schizophrenia in our hospital from April 2012 to December 2013 were randomly divided into treatment group,hyperbaric oxygen group,research group and 35 cases in each group.The treatment group was treated with risperidone,the hyperbaric oxygen group was treated with hy- perbaric oxygen therapy,the research group was treated with risperidone combined with hyperbaric oxygen therapy.Ad- mission of 4,8,12 weekends positive and negative symptoms scale （PANSS） and adverse reactions and the course of the disease,and followed up for 1 year recurrence rate and total health expenses among three groups was observed. Results The total effectiveness of treatment group was 45.7%,the total effective rate of hyperbaric oxygen group was 40.0%,the total effective rate of research group was 77.1%,the total effective rate among the 3 groups was statistically significant （P〈0.05）.The team course,medical cost,recurrence rate of treatment group was lower than that of the other two groups respectively （P〈0.05）.The TESS total score of research group in the 4 weekend,8 weekend,12 weekend was lower than that of the remaining two groups respectively （P〈0.01）;At the same time,the adverse reaction was given priority to with department of vertebral body symptoms;In the degree it was treatment group,the research group,the hyperbaric oxygen group,the number of adverse reactions in the treatment group and research group were 19 cases （54.3%） and 9 cases （25.7%）.At the end of 12 weekend,the total score of PANSS,negative factor score and PANSS negative factor scores of research group was significantly lower than that of the same group respectively,and was lower than that in the other groups at the same time （P〈0.05）. Condusion Hyperbaric oxygen treatment is given priority to with negative symptoms of schizophrenia patients,the result is safe and effective.And it combined with risperidone,can effectively improve the clini-cal efficacy,reduce adverse reactions,shorten the course of disease,reduce the recurrence rate,medical expenses.
