慢性肾功能不全患者二尖瓣瓣环位移值与肾小球滤过率的关系分析

摘要 目的探讨慢性肾功能不全(CKD)患者二尖瓣瓣环位移(TMAD)值与肾小球滤过率(GFR)的关系。方法选取本院2014年12月~2015年7月收治的118例慢性肾功能不全并且经常规二维双平面Simpson法测量左室收缩功能(LVEF)≥50%的患者作为病例组,选取2015年2~4月的30例体检正常者作为对照组。病例组根据GFR水平从高到低分成4组。采用二维斑点追踪显像技术(STI)测量TMAD值。比较各组的TMAD值,分析GFR与TMAD值间的关系。结果 4个病例组的TMADmid值显著低于对照组,差异有统计学意义(P〈0.05)。4个病例组间的TMADmid值比较,差异有统计学意义(P〈0.05)。TMADmid值随GFR的减低呈下降趋势。结论TMAD技术能够早期、准确地评价慢性肾功能不全患者GFR与左室收缩功能的关系,是一种简单、可行的检查左室收缩功能的新方法。 Objective To explore the relationship of tissue motion mitral annular displacement (TMAD) value and glomeru- lar filtration rate (GFR) in patient with chronic renal insufficiency (CKD). Methods 118 patients who were definitely di- agnosed as CKD and left ventricular systolic function (LVEF) ≥ 50% by conventional two-dimensional biplane Simpson method from December 2014 to July 2015 in our hospital were selected as the disease group,30 cases of normal physi- cal examination from February 2015 to April were selected as the control group.In the disease group,the cases were di- vided into 4 groups according to the level of GFR from high to low.The TMAD value was measured by Speckle-track- ing imaging (STI).The TMAD value among each group was compared,and the relationship between GFR and TMAD value was analyzed. Results The TMADmid values among 4 groups was lower than that in the control group,with signifi- cant difference (P〈0.05).There was significant difference in TMADmin value of 4 disease groups (P〈0.05).The TMADmid value decreased with the decrease of GFR. Conclusion TMAD can evaluate the correlation between GFR in CKD pa- tients and left ventricular systolic function at an early stage and accurately,which is a simple and feasible new method to examine left ventricular systolic function.
