解剖型锁定钢板治疗跟骨骨折的临床效果观察

Clinical effect observation of anatomic locking plate treating calcaneal fracture

目的 观察解剖型锁定钢板治疗跟骨骨折的临床效果。方法 选取我院2012年1月~2014年11月收治的96例跟骨骨折患者作为研究对象,随机分为观察组和对照组,各48例。对照组采用传统钢板内固定治疗,观察组采用解剖型锁定钢板内固定。比较两组患者的手术时间、住院时间、骨折愈合时间、跟骨功能恢复正常时间,对患者的术后足部功能进行评分,记录术后并发症发生情况。结果 手术后,对照组患者的手术时间、住院时间分别为（68.2±20.5）min、（9.8±2.2）d,均长于观察组的（56.9±12.6）min、（7.1±1.8）d,差异有统计学意义（P〈0.05）;对照组患者的骨折愈合时间、跟骨功能恢复正常时间分别为（51.6±8.3）、（119.2±18.7）d,均长于观察组的（46.7±7.5）、（78.2±13.5）d,差异有统计学意义（P〈0.05）;观察组的总有效率为91.7%,明显高于对照组的68.8%（P〈0.05）;对照组的术后并发症发生率为16.7%,高于观察组的4.2%,差异有统计学意义（P〈0.05）。结论 解剖型锁定钢板固定方法治疗跟骨骨折的效果良好,能明显改善患者的足部功能,且并发症少,安全可靠,值得临床推广应用。

Objective To observe the clinical effect of anatomic locking plate treating calcaneal fracture. Methods 96 pa- tients with calcaneal fracture treated by out hospital from January 2012 to November 2014 were selected as study ob- ject.Patients were randomly divided into observation group and control group,and each group was 48 cases.Control group was treated by traditional plate internal fixation,while observation group was treated by anatomic locking plate in- ternal fixation.The operation time,hospital stay,fracture healing time and the time of calcaneal function returning to nor- mal in patients between two groups was compared respectively.The score of foot function was evaluated after operation and the incidence of postoperative complication was recorded. Results After operation,the operation time and hospital stay in control group [（68.2±20.5） min,（9.8±2.2） d] was longer than that that of observation group [（56.9±12.6） min,（7.1± 1.8） d] respectively,and the difference was statistical significance （P〈0.05）;the fracture healing time and the time of cal- caneal function returning to normal time in control group [（51.6±8.3） d,（119.2±18.7） d]was longer than that of observa- tion group [（46.7±7.5）,（78.2±13.5）d] respectively,and the difference was statistical significance （P〈0.05）;the total effec- tive rate in observation group （91.7%） was obviously higher than that of control group （68.8%） （P〈0.05）.The incidence rate of postoperative complication in control group （16.7%） was higher than that of observation group （4.2%）,and the difference was statistical significance （P〈0.05）. Conclusion Anatomic locking plate treating calcaneal fracture has a good curative effect,and it can improve the foot function of patients obviously,with fewer complication,it is safe and reli- able.It is worthy of clinical promotion and application.
