放疗后牙周组织改变的短期观察研究 被引量:3

摘要 目的:口腔内软硬组织损伤是头颈部放疗的主要并发症之一。本研究旨在探讨因头颈部恶性肿瘤接受的放射治疗对患者牙周组织的损伤作用。方法:收集2013年7月-2015年2月因头颈恶性肿瘤,拟行涉及颌面部范围内放射治疗的患者。在患者放疗前和放疗后6个月两个时间点检测患者照射范围内牙齿的牙周参数,包括探诊牙周袋深度、临床附着水平、牙龈退缩等,通过配对t检验进行统计学分析对比放疗前后的变化。结果:共25例符合标准患者被纳入本研究,发现放疗前后牙周袋深度无明显变化(P=0.892),而放疗后临床附着丧失和牙龈退缩较放疗前均明显增加(P=0.035、0.044)。结论:放疗导致牙周组织附着丧失和牙龈退缩,在放疗前后及放疗过程中均应注意牙周组织的健康。 Objective:Hard and soft oral tissue injury is a major complication of radiotherapy for head and neck.This study is designed to investigate the injury effect of periodontal tissue due to head and neck cancer patients receiving radiation therapy.Method:Patients who underwent radiation therapy due to head and neck cancer were recruited from July 2013 to February 2015 in our hospital.Periodontal parameters including probing pocket depth,clinical attachment level and gingival recession were detected before and 6 months after radiotherapy.The changes were statistically analyzed and compared by paired t-test before and after radiotherapy.Result:A total of 25 patients met the criteria were included to this study.We found no significant changes in the periodontal pocket depth before and after radiotherapy(P=0.892),however 6 months after radiotherapy,the clinical attachment loss and gingival recession significantly increased compared with those before radiotherapy(P=0.035,0.044).Conclusion:Radiotherapy cause periodontal tissue attachment loss and gingival recession.We should pay attention to the health of periodontal tissue before and after radiotherapy.
