血府逐瘀汤对慢性阻塞性肺疾病急性加重合并肺动脉高压患者肺部感染控制窗的影响

Effect of Xuefu Zhuyu Decoction on Pulmonary Infection Control Window in Patients with AECOPD Complicated by Pulmonary Hypertension

目的观察血府逐瘀汤对慢性阻塞性肺疾病(COPD)合并肺动脉高压患者肺部感染控制窗以及机械通气时间的影响。方法将COPD急性加重(AECOPD)合并肺动脉高压重症患者40例按随机数字表法分为治疗组与对照组,对照组单纯采用西医治疗,治疗组在对照组基础上加用血府逐瘀汤治疗。两组疗程均为7 d。结果治疗组在抗生素使用、感染控制窗、机械通气时间方面均短于对照组(P<0.05);两组在住院时间、二重感染发生率方面差异无统计学意义(P>0.05)。两组治疗后中医证候评分、纤维蛋白原及D-二聚体水平均较治疗前下降(P<0.05);两组比较,治疗组下降更加明显(P<0.05)。结论使用血府逐瘀汤治疗AECOPD合并肺动脉高压患者较单纯西药能明显缩短抗生素使用时间和机械通气时间。但两组在住院时间、二重感染发生率方面差异效果相当(P>0.05)。

Objective： To observe the effect of the Xuefu Zhuyu Decoction on pulmonary infection control window and mechanical ventilation time in patients with AECOPD complicated by pulmonary hypertension. Methods： 40 patients with pulmonary hypertension were randomly divided into two groups. The treatment group with Xuefu Zhuyu Decoction was treated in AECOPD combined with pulmonary hypertension,and the control group was treated with western medicine. The differences of TCM syndromes score,time of antibiotics and mechanical ventilation,length of hospitalization time,the incidence of super infection,and the differences between the two groups of fibrinogen and D-dimer were compared. Results： There were significant differences in the time of use of antibiotics,infection control window time and mechanical ventilation time between the two groups（P〈 0.05）,but there was no significant difference in length of hospital stay and the incidence of super infection（P〉 0.05）.The TCM syndrome scores,the fibrinogen and D-dimer of the two groups were lower than those before treatment;patients in the treatment group were better than those in the control group,and the difference was statistically significant（P〈 0.05）. Conclusion： Compared those with simple western medicine patients taking Xuefu Zhuyu Decoction significantly shorten the time of use of antibiotics and mechanical ventilation time. But there was no significant difference in hospitalization time and super infection rate between the two groups.
