摘要
【目的】肱踝脉搏波传导速度(ba PWV)和踝臂指数(ABI)是评价动脉硬度的理想指标。本研究拟探讨牙周非手术治疗对中重度牙周炎患者ba PWV、ABI和可溶性细胞间黏附分子(s ICAM-1)的影响。【方法】选择单纯中重度牙周炎患者98例,随机分配至实验组(n=49)进行牙周非手术治疗;对照组(n=49)进行龈上洁治术。分别在治疗前2周和治疗后3个月测量牙周指标、ba PWV、ABI和血清s ICAM-1等。【结果】牙周治疗后3个月,实验组ba PWV和s ICAM-1与治疗前显著减低(P<0.001),且明显低于对照组(P<0.05);ABI较治疗前有所升高,但差异不具有统计学意义(P>0.05)。实验组ba PWV与牙周探诊深度PD变化量呈正相关(r=0.858,P<0.001)。【结论】牙周治疗有助于改善动脉硬度和降低血清s ICAM-1水平,对降低心血管风险具有一定的作用。
【Objective】 Brachial-ankle pulse wave velocity(ba PWV) and ankle-brachia index(ABI) are thought to be ideal indexes for the assessment of arterial stiffness. This study aimed to explore the effects of non-surgical periodontal therapy on ba PWV,ABI and s ICAM-1 with moderate or severe periodontitis. 【Methods】 A total of 98 patients with moderate to severe periodontitis were recruited and randomized into experiental group(n = 49) who received non-surgical periodontal treatment and control group(n = 49)who received supragingival scaling only. All parameters, including periodontal clinic index, ba PWV, ABI and serum s ICAM-1, were examined at 2 weeks prior to treatment and 3 month follow-up. 【Results】 Three month after treatment, the experimental group demonstrated significant decreases in ba PWV and serum s ICAM-1, compared with baseline(P〈0.001) and with the control group(P〈0.05). There was an increase in ABI, but it did not reach statistical significance(P〈0.05). Positive correlations were found between the changes of ba PWV and PD in the experimental group(r = 0.858, P〈0.001). 【Conclusion】 This study demonstrated that non-surgical periodontal therapy can improve arterial stiffness and reduce serum s ICAM-1 of patients with moderate or severe periodontitis, which might be helpful for decreasing the risk of CVD.
