针刺单穴和组穴的fMRI研究 被引量:11

fMRI study of acupuncture with single and unitized acupoints

摘要 目的:采用局部一致性(ReHo)方法,探讨针刺健康受试者单穴和组穴后静息态下脑活动变化。方法:57名健康受试者随机分为三组,分别接受针刺单穴(右侧百劳)、组穴一(右侧百劳+右侧合谷)、组穴二(右侧百劳+右侧中渚),分别于针刺前、针刺后采集静息态功能磁共振数据,应用局部一致性方法对fMRI数据进行处理。结果:与针刺前相比,单穴组针刺后右侧中央前回的ReHo值降低,组穴一针刺后右侧扣带回中部ReHo值增高且右侧中央前回、中央后回ReHo值减低,组穴二针刺后左侧前扣带回、左侧额上回内侧、右侧中央前回、右侧中央后回的ReHo值增高且左侧后扣带回、双侧辅助运动区的ReHo值降低。与单穴组相比,针刺组穴一导致双侧额上回内侧、右侧中央后回ReHo值减低;与单穴组相比,针刺组穴二导致双侧额上回内侧的ReHo值减低且右侧中央前回、右侧中央后回的ReHo值增高。结论:针刺单穴、组穴可引起不同脑区的局部一致性改变,两者具有相似性,又具有一定的差异性。针刺组穴可引起更多的脑区发生改变,涉及到感觉、运动、疼痛矩阵等多个方面。 Objective:To study the brain activity changes of healthy subjects in resting state after receiving acupuncture of single and unitized acupoints by using regional homogeneity(ReHo)method.Methods:Fifty-seven healthy subjects were randomly assigned into three groups including single acupoint group(right EX-HN15),group 1of unitized acupoints(right EX-HN15,right LI4)and group 2of unitized acupoints(right EX-HN15 and right SJ3).Functional magnetic resonance imaging(fMRI)data were collected before and after acupuncture respectively and analyzed by ReHO method.Results:Compared with the data collected before acupuncture,the ReHo values decreased in the right precentral gyrus for those who received acupuncture of single acupoint.For group 1of unitized acupoints,the ReHo values increased in right middle cingulate gyrus and decreased in right precentral gyrus and posterior central gyrus.For group 2with unitized acupoints,the ReHo values increased in the left anterior cingulate gyrus,left superior medial frontal gyrus,right precentral gyrus and right postcentral gyrus,while decreased in left posterior cingulate gyrus and bilateral supplementary motor areas.Compared with the group of single acupoint,the ReHo values were lower in bilateral superior medial frontal gyrus and right postcentral gyrus for the group 1with unitized acupoints,as well as in the bilateral superior medial frontal gyrus for the group 2of unitized acupoints.However,ReHo values were higher in the right postcentral gyrus for the group 2of unitized acupoints than those for the group of single acupoint.Conclusion:There are similarities in the changes of ReHo values which caused by acupuncture of single acupoint or grouped acupoints in different brain regions,yet with discrepancy as well.In addition,the results suggest that acupuncture at the unitized acupoints might affect more brain regions,such as somatosensory system,motor,pain matrix etc.
