摘要
目的:评估胃癌的ADC值与组织病理学因素的相关性。方法:回顾性分析68例胃癌患者的MRI资料,DWI b值取0和800s/mm^2。术后组织病理学因素包括肿瘤部位、TNM分期、组织学类型。分析肿瘤ADC值与各组织病理学因素之间的相关性。应用ROC曲线确定不同TNM分期和组织学类型的ADC鉴别界值。结果:随着胃癌TNM分期的增加,分化程度的减低,肿瘤的平均ADC值呈递减趋势。肿瘤的不同TNM分期、组织学类型和发生部位的ADC值差异具有统计学意义。诊断TNMⅠ期、Ⅱ期和高分化腺癌的ADC鉴别界值分别为0.917、0.906和1.034×10^(-3)mm^2/s,ROC曲线下面积分别为0.702、0.795和0.960。结论:ADC值作为一个无创性生物学指标有助于评估胃癌的组织病理学因素,包括TNM分期、组织学类型和发生部位。
Objective:To evaluate the correlation between histology features and apparent diffusion coefficient(ADC)value on diffusion-weighted imaging(DWI)of gastric cancer.Methods:MRI materials of 68 cases with gastric cancer were analyzed retrospectively.The b values were 0,800s/mm2 on DWI.Post-surgical pathologic features included tumor site,TNM staging and histology pattern.Correlation between ADC values and histology features were analyzed.Receiver operating characteristic curve(ROC)analysis was performed to find out the diagnostic cutoff of the ADC value,to distinguish different TNM staging and histology pattern of gastric cancer.Results:As the TNM stage increased,the mean ADC values of gastric cancer showed a trend of decreasing.There were significant differences in the mean ADC values among gastric cancer with different TNM stageing,histology pattern and tumor site.The diagnostic cutoff of the ADC values used to distinguish TNM stageⅠ,stageⅡ and well differentiated adenocarcinaoma was 0.917,0.906,and 1.034×10-3 mm2/s respectively,the area under the ROC curve was 0.702,0.795 and 0.960 respectively.Conclusion:The ADC values as a noninvasive biology parameter,might be helpful for evaluating the histological features of gastric cancer,including TNM staging histology pattern and tumor site.
