延续护理模式在消化道出血介入治疗后贫血患者中的应用

Application of continuing care mode in nursing patients with anemia after interventional treatment for gastrointestinal bleeding

目的探索一条应用于消化道出血介入治疗后贫血患者延续护理的有效途径。方法基于网络信息平台的构建,采用电话随访、微信平台、腾讯QQ和手机短信等信息传递方式,对消化道出血介入治疗后贫血的患者实施系统的规范性延续护理,比较干预前、中、后患者乏力程度和血细胞的变化情况。结果实施延续护理模式后,患者贫血状况得到改善,表现为乏力程度(χ2=45.06,P=0.000)逐渐降低,且重度以上贫血患者逐渐减少;干预1个月后患者红细胞(RBC)(t=-3.12,P=0.004)和血红蛋白(HGB)含量(t=-2.08,P=0.047)显著升高;而干预6个月后只有RBC计数表现出逐步升高的趋势(t=3.62,P=0.042)。结论基于网络信息平台的延续护理模式适用于消化道出血且行介入治疗的患者,能在一定程度上改善患者的贫血状况,值得推广使用。

Objective To explore an effective way that can be used in clinical nursing for patients with anemia after interventional treatment of digestive tract hemorrhage. Methods Based on the construction of network information platform, by using telephone follow-up, WeChat platform, Teneent QQ and mobile phone text messages and other information transfer mode, the systemic and normative continuing care was implemented for patients with anemia after interventional treatment of digestive tract hemorrhage. The degree of fatigue and the changes of blood cells before and after the intervention were compared. Results After the implementation of the continuing care mode, the anemia was improved, the fatigue degree became gradually decreased （χ^2=45.06, P=0.000）, and the number of patients with severe anemia was gradually reduced. After one month of intervention, both red blood cell （RBC） count and hemoglobin （HGB） content were significantly increased （t=-3.12, P=0.004 and t=-2.08, P=0.047 respectively）. After 6 months of intervention, only RBC count showed gradually increasing trend （t=3.62, P=0.042）. Conclusion Continuing care mode that based on network information platform is suitable for the nursing of patients with anemia after interventional treatment of digestive tract hemorrhage, which can improve the patient＇s anemia in a certain extent, and this nursing mode is worth promoting.
