基本药物制度对基层医疗机构经济运行情况的影响——基于4省6地的调查

Analysis on the impact of essential medicine system to the economic operation status in primary health institutions——Based on the investigation of 4 provinces and 6 places

[目的]探讨基本药物制度的实施对基层医疗机构经济运行情况的影响。[方法]按照东中西部地区,采用典型抽样的方法,抽取浙江、江西、陕西和新疆4省(自治区),各地基层医疗机构作为研究对象,收集2009-2012年的各项收支相关的数据进行分析,同时对相关人员进行访谈。[结果]基本药物制度实施后,基层医疗机构总收入及财政补助收入均有所上升,财政补助收入占比明显上升,同时药品收入占比下降。[结论]基本药物制度的实施对基层医疗机构的收入及其结构影响较大,补偿制度及激励考核措施有待完善。

Objective To study impacts of essential medicine system to economic operation status in primary health institutions. Methods Selected Zhejiang,Jiangxi,Shaanxi and Xinjiang four provinces as the research objects by using the method of typical sampling,selected primary health institutions as the research objects. Collected and analyzed the relevant data of income and expenses from 2009 to 2012,interviewed with relevant personnel at the same time. Results After the implementation of the essential medicine system,total income and financial subsidy income had risen. The percentage of financial subsidies income increased significantly. While the percentage of drug income dropped significantly.Conclusions The implementation of essential medicine system has large effect to the income and the structure of primary health institutions.Compensation system and motivation assessment system has yet to be perfected.
