降钙素原与C-反应蛋白联合检测在革兰阴性杆菌血流感染脓毒症患者中的临床分析 被引量:26

Clinical effect of combined detection of procalcitonin and C-reactive protein on sepsis patients with gram-negative bacilli bloodstream infections

摘要 目的研究革兰阴性杆菌血流感染脓毒症患者采用降钙素原(PCT)及C-反应蛋白(CRP)联合检测的临床效果,以提高革兰阴性菌感染的诊断意义。方法选取2013年4月-2015年4月在医院进行治疗的革兰阴性杆菌血流感染脓毒症患者共80例为试验组,同期选取80例其他局部感染者作为对照组,对比两组患者临床指标,数据采用SPSS 16.0软件进行统计分析。结果试验组患者PCT、CRP水平及PCT、CRP阳性率均高于对照组,差异有统计学意义(P<0.05);CRP联合PCT进行检测的特异性、敏感度、阳性预测值、约登指数以及阴性预测值分别为91.82%、79.10%、90.42%、70.53%、81.29%。结论 CRP联合PCT对革兰阴性杆菌血流感染脓毒症者进行检测的诊断价值比较高,使诊断效能提升。 OBJECTIVE To observe the clinical effect of combined detection of procalcitonin(PCT)and C-reactive protein(CRP)on sepsis patients with gram-negative bacilli bloodstream infections so as to improve the clinical significance in diagnosis of the gram-negative bacteria infections.METHODS A total of 80 sepsis patients with gramnegative bacilli bloodstream infections who were treated in the hospital from Apr 2013 to Apr 2015 were assigned as the experimental group,meanwhile,80 patients with other local infections were set as the control group.The clinical indicators were observed and compared between the two groups of patients,and the statistical analysis of data was performed with the use of SPSS 16.0software.RESULTS The levels of PCT and CRP were higher in the experimental group than in the control group;the positive rates of PCT and CRP were higher in the experimental group than in the control group,and there was significant difference(P0.05).The specificity of the combined detection of CRP and PCT was 91.82%,the sensitivity 79.10%,the positive predictive value 90.42%,the Youden index 70.53%,the negative predictive value 81.29%.CONCLUSIONThe combined detection of CRP and PCT has high value in diagnosis of the sepsis patients with gram-negative bacilli bloodstream infections,and it can improve the effectiveness of diagnosis.
