超声引导在颈内静脉置管中的应用价值

Clinical Application of Ultrasound-Guided Internal Jugular Vein Catheterization

【目的】探讨超声引导在颈内静脉穿刺置管中的临床应用价值。【方法】回顾分析本院2008年1月至2014年4月期间行颈内静脉置管的292例患者的临床资料。将86例未行超声引导进行颈内静脉置管者作为A组，其中男38例，女48例，平均年龄（45．5±6．7）岁；将206例行超声引导进行颈内静脉置管者作为B组，其中男108例，女98例，平均年龄（46．7土6．9）岁，比较两组穿刺成功率、置管成功率、穿刺时间及穿刺并发症发生情况。【结果】与A组相比较，B组一次性穿刺成功率、置管成功率高，穿刺时间短，并发症发生率低，且两组相比较差异有显著性（PdO．05）。【结论】超声引导行颈内静脉穿刺置管操作简单、方便、安全，值得临床应用推广。

[Objective]To explore the experience of ultrasound-guided internal jugular vein catheterization. [Methods]The clinical data of 292 cases from January 2008 to April 2014 were analyzed retrospectively. All patients were divided into an A group and B group. The A group did not use ultrasound-guided internal jugular vein catheterization （86 cases, 38 male and 48 female, mean age 45.5 ± 6.7 years）. The B group used ultrasound-guided internal jugular vein catheterization. （206 cases , 108 male and 98 female, mean age 46.7 ± 6.9 years）. The total success rate, first time intubation success rate, puncture time, and complication incidence were recorded and compared. [Results]Group 13 had higher total success rate, first time intubation success rate, low incidence rate of postoperative complication, and needed shorter puncture time than group A. [Conclusion]Ultrasound-guided internal jugular vein catheterization has the advantages of simple operation, convenience, and safety. It is deserving of clinical application and usage.
