术中保温对妇科腹腔镜手术患者体温及凝血功能的影响 被引量:25

摘要 目的探讨术中保温对妇科腹腔镜手术患者的体温、凝血功能及术后并发症的影响。方法将120例择期腹腔镜手术的妇科肿瘤患者分为两组,对照组(60例)患者按常规行腹腔镜手术;干预组(60例)在其基础上加电子加温毯、充气加热气囊及保温帽。比较两组患者的温度、凝血功能、平均动脉压、心率、寒战和躁动情况。结果对照组和干预组低温发生率分别为53.3%(32/60)和5.0%(3/60),两组比较,差异有统计学意义(P<0.05);对照组凝血酶原时间(PT)在手术60min后[(15.00±0.54)s],活化部分凝血活酶时间(APTT)在手术结束时异常[(45.36±2.33)s],凝血酶时间(TT)手术30min异常[(19.12±1.11)s],干预组在正常值范围内,两组比较,差异有统计学意义(P<0.05);对照组术中平均动脉压、心率波动、躁动、寒战发生率与干预组比较,差异均有统计学意义(P<0.05);单因素和logistic分析显示,全身麻醉时间和CO_2量是术中低温的独立危险因素。结论妇科腹腔镜手术术中保温可以有效维持患者体温及凝血功能,减少术中及术后并发症。
