532例哮喘患者的气道炎症表型及临床特征分析 被引量:15

摘要 目的探讨哮喘患者的气道炎症表型分布及临床特征。方法选取532例支气管哮喘患者,根据其诱导痰细胞学结果进行气道炎症分型,比较重症组、普通组哮喘患者的气道炎症表型分布及不同气道炎症表型的重症患者的呼出气一氧化氮(Fe NO)水平及肺功能。结果 532例患者中嗜酸性粒细胞型最为常见,占47.0%,其次是混合细胞型(27.3%)、中性粒细胞型(21.4%)及寡细胞型(4.3%)。重症组诱导痰嗜酸性粒细胞型百分比高于普通组(P<0.05)。中性粒细胞型重症患者的第1秒用力呼气容积(FEV1)、FEV1/用力肺活量及FEV1占预计值的百分比均显著低于其他类型重症患者,且其Fe NO显著低于嗜酸性粒细胞型及混合细胞型(P均<0.05)。结论哮喘气道炎症表型包括嗜酸性粒细胞型、混合细胞型、中性粒细胞型及寡细胞型,其中嗜酸性粒细胞型最常见;重症哮喘患者中中性粒细胞型的肺功能最差。
