103例乳腺癌规范化保乳治疗的效果分析

Cllinical effects of standardized breast-conserving therapy in 103 primary breast cancer

目的 探讨乳腺癌规范化保乳治疗的临床疗效。方法 103例原发性乳腺癌接受局部扩大切除术＋前哨淋巴结活检或腋窝淋巴结清扫术治疗,手术后患侧乳房接受放射治疗。回顾性分析患者的临床和病理资料,随访监测局部复发或新发及远处转移情况。结果 103例患者保乳手术顺利,术后随访外形满意。中位随访时间49（3~112）个月,4例同侧乳腺局部复发,其中3年内局部复发1例（1.0%）,3~4年局部复发3例（2.9%）。新发乳腺癌2例（1.9%）,远处转移2例（1.9%）,无死亡病例。结论 规范化保乳手术治疗乳腺癌的临床疗效满意,可作为0期及Ⅰ~Ⅱ期乳腺癌的首选治疗方法。

Objective To evaluate the clinical effects of standardized breast-conserving therapy in primary breast cancer patients. Methods A total of 103 cases of primary breast cancer received local extended resection plus sentinel lymph node biopsy or axillary lymph node dissection treatment, and ipsilateral breast radiation therapy was implemented after surgery. The clinical and pathological data were reviewed, and the follow-up information on local recurrence, distant metastasis or new occurrence of breast cancer were also reviewed. Results All of the patients underwent breast-conserv- ing surgery successfully, and they were satisfied about postoperative appearance of breast. After a median of 49 months of follow-up （range 3-112 months）, 4 cases of local recurrence were observed in the ipsilateral breast, of which, 1 case （1.0%） occurred within 3 years after surgery, and the remaining 3 cases （2.9%） occurred between 3-4 years after surgery. Two cases （1,9%） of newly diagnosed breast cancer and 2 cases （1.9%） of distant metastases occurred respectively, and no cases of death were observed. Conclusion Clinical effects of standardized breast-conserving treatment is satisfactory for stage 0 and Ⅰ - Ⅱ breast cancer.
