摘要
目的:观察中医综合干预对术后胃肠功能障碍患者预后与炎症因子的影响。方法:选取2010年11月—2013年8月入住本院外科做腹部手术后胃肠功能障碍患者180例,随机分为治疗组与对照组,每组90例。所有患者术后均给予抗感染治疗,术后12 h拔除胃管,拔胃管后进食清流质,术后24 h内嘱患者适当活动,活动下肢,使用促进胃肠功能药物,治疗组另给予中医综合干预治疗。结果:对照组有效率为90.0%,治疗组有效率为98.9%,治疗组优于对照组(P<0.05);治疗组肛门排气时间、肛门排便时间、肠鸣音恢复时间均优于对照组,差异有统计学意义(P<0.05);治疗组干预后胃肠功能评分、血清CRP水平均优于对照组(P<0.05)。结论:中医综合干预能有效恢复腹部手术后胃肠功能障碍患者胃肠功能,减少应激反应,其作用可能是通过减轻炎症因子的表达有关。
Objective: To observe the influence of Chinese medicine intervention on the prognosis and inflammatory factors in postoperative patients with gastrointestinal disorder. Methods: Totally 180 patients with gastrointestinal disorder after abdominal operation hospitalized to the surgery department between November 2010 and August 2013 were randomly divided into treatment group and control group,with 90 cases in each group. All postoperative patients were given anti-infection treatment,removed the stomach tube 12 hours after the operation and then fed with clear liquid food. Within the 24 hours after the operation,the patients were advised to do proper exercises especially the exercises for the lower limbs,and they were administered with drugs which can promote gastrointestinal function. Treatment group were additionally given Chinese medicine intervention. Results: The effective rate was98. 9% in treatment group,better than the 90. 0% in control group( P 〈 0. 05). The time for anal evacuation and defecation,as well as recovery time for borborygmus in treatment group was better than that in control group; the difference was statistically significant( P 〈 0. 05). And the scores of gastrointestinal function and levels of serum CRP were better than these in control group( P 〈 0. 05). Conclusion: Chinese medicine intervention can effectively restore the gastrointestinal function in patients after abdominal operation,reduce the stress reaction,whose action may be related with the reduction in the expression of inflammatory factors.
