摘要
目的:探讨米力农在治疗难治性心力衰竭(RHF)中的应用及疗效。方法:将RHF患者分为对照组(A组,62例)和试验组(B组,63例),A组采用常规治疗,而B组采用在常规治疗的基础上,给予50μg/kg米力农静脉注射后,给予0.5μg/(kg·min)持续24 h泵入,连用7 d。统计两组治疗前后的超声心功能指标(SV、CO、CI、LVEF、LVEDD、VE、VA)、6 min步行试验距离、炎性因子(TNF-α、IL-6、IL-1β)、血清N-末端B型利钠肽前体(NT-Pro BNP)、去甲肾上腺素(NE)、基质金属蛋白酶-9(MMP-9)的数据,利用χ^2检验、t检验和t'检验分析相关数据。结果:治疗后,CO、LVEF、LVEDD、VA、TNF-α、IL-6、IL-1β、NT-Pro BNP和NE在两组中均有明显改善,但B组改善的程度更加明显,差异有统计学意义(P〈0.05)。治疗后,B组的SV、CI、VE、6 min步行距离和MMP-9明显比治疗前好转,而且也明显好于治疗后的A组,差异有统计学意义(P〈0.05)。结论:米力农在改善RHF患者血流动力学等方面的效果非常明显,值得推荐短期使用。
Objective To investigate the application and curative effect of Milrinone in the treatment of refractory heart failure( RHF). Method RHF patients were divided into control group( 62 cases) and experimental group( 63 cases). Treatment protocols of control group were standard therapies,and experimental group were continuous 24 h pumped milrinone( 0. 5 μg/kg ·min) after single intravenous milrinone( 50 μg/kg),which the course of treatment was 7 days. The index of ultrasonic cardiac function( SV,CO,CI,LVEF,LVEDD,VE,VA),distance of floor walking test in 6 minute,inflammatory factor( TNF- α,IL- 6,IL- 1β),N- terminal pro- B type natriuretic peptide precursor( NT- Pro BNP),norepinephrine( NE),matrix metalloproteinases- 9( MMP- 9) were be statistical,using chi- square test,t test and t' test to analyze the data. Results The CO,LVEF,LVEDD,VA,TNF- α,IL- 6,IL- 1β,NT- Pro BNP and NE all improve noticeably in the two groups after treatment,and The degree of improvement was more obvious in Group B,the difference was statistically significont( P〈0. 05). After treatment,SV,CI,VE,distance of floor walking test in 6 minute and MMP- 9 were more improved than before treatment,and also improved significantly than after treatment group A' s,the difference was statistically significont( P〈0. 05). Conclusion The therapeutic effect is clear- cut which use milrinone to treat RHF in hemodynamics and so on,so it is worth to recommend short- term use.
