评估下腔静脉扩张指数在重型颅脑外伤患者中的临床价值 被引量:4

摘要 目的:结合唯捷流技术(Vigileo),探讨床旁超声测量下腔静脉扩张指数(distensibility index of the inferior vena cava,d IVC)在重型颅脑外伤患者中容量反应性(volume responsiveness)的评估作用。方法:41例重型颅脑外伤患者行被动抬腿试验(passive leg raise,PLR test)共计61例次,均给予呼吸机辅助呼吸,入科后都予留置桡动脉行血流动力学监测唯捷流技术(Vigileo Edawards Lifesciences,CA,USA),记录PLR前后的每搏输出量(stroke volume,SV)变化,计算出SV变化值(△SV),以△SV≥15%为预测容量反应性阳性,分析采用床旁超声测量d IVC评估重型颅脑外伤患者容量反应性的价值。结果:有反应组PLR前后的血流动力学变化有统计学意义(P<0.05),无反应组患者的PLR前后的血流动力学变化无统计学意义(P>0.05)。有反应组△SV和d IVC与无反应组比较,差异有统计学意义(P<0.05)。d IVC预测容量反应性的受试者工作特征曲线(ROC)下面积(AUC)为0.422。结论:下腔静脉扩张度在评估重型颅脑外伤患者容量反应性方面有一定的应用价值。
