不同手术方式治疗子宫肌瘤对患者术后妊娠的影响

Influence of different surgical treatment of uterine fibroids on postoperative patients with pregnancy

目的探讨不同手术方式治疗子宫肌瘤对患者术后妊娠的影响。方法选取2013年2月至2014年8月间收治的120子宫肌瘤患者,按照随机数字表法分为观察组和对照组,每组60例。观察组患者采用腹腔镜手术,对照组患者采用传统开腹手术。观察两组患者治疗有效率、术后粘连发生率、妊娠率及病情复发率等,分析两种手术方式对患者妊娠的影响。结果观察组患者的治疗有效率和妊娠率分别为89.3%和68.3%,对照组则分别为43.3%和23.3%,观察组患者明显高于对照组(P<0.05)。观察组患者术后粘连率和复发率分别为6.7%和8.3%,对照组则分别为38.3%和41.7%,观察组患者术后粘连率和复发率明显低于对照组,差异有统计学意义(P<0.05)。结论腹腔镜手术比传统手术方式具有更好的治疗效果,能有效降低术后粘连和复发率,提高患者妊娠率,满足患者妊娠需求,值得临床推广。

Objective To investigate influence of different surgical treatment of uterine fibroids different on postoperative patients with pregnancy. Methods From February 2013 to August 2014 in Chengdu Women and Children＇s Hospital,120 uterine fibroids patients were enroll in the study and according to the different operation methods,they were divided into the observation group and the control group,60 cases in each group. The patients in the observation group accepted laparoscopic surgery treatment and those in the control group accepted traditional open operation. The effective rate,incidence of postoperative adhesion,pregnancy rate and disease recurrence rate between the two groups were compared. Results After followuped for 1 mo - 3 yr,the treatment efficient and pregnancy rates of the observation group were 89. 3%,68. 3%,and the control group were 43. 3%,43. 3%,with significant differences（ P〈0. 05）. The postoperative adhesion rate,recurrence rate of the observation group were 6. 7%,8. 3%,and the control group were 38. 3%,41. 7%,with significant differences（ P〈0. 05）. Conclusions Laparoscopic surgery has better therapeutic effects than traditional surgery way,and can effectively reduce postoperative adhesion and the recurrence rate,increase the rate of patients with pregnancy,meet the demand of patients with pregnancy. And it is worthy of popularization and application clinically.
