摘要
目的:评价桑菊饮联合诱导排痰治疗小儿风热咳嗽的临床疗效及作用机制。方法:将确诊为风热咳嗽的90例患儿随机分为痰诱导组45人和中药加痰诱导组45人。2组均予以高渗盐水雾化排痰,中药加痰诱导组加用桑菊饮口服。观察治疗前及治疗7d后患儿症状体征的变化。同时对痰标本炎性细胞进行分类计数,ELISA法检查诱导痰上清液中IL-6的含量。结果:与痰诱导组相比,中药加痰诱导组咳嗽情况、咽部红肿情况、睡眠情况、流涕症状、肺部湿啰音吸收程度、舌苔、脉象明显好转,差异具有统计学意义(P<0.01)。中药加痰诱导组治疗后患儿痰液中性粒细胞比例、嗜酸性粒细胞比例、巨噬细胞比例均显著低于痰诱导组,诱导痰液上清中IL-6水平显著降低,差异具有统计学意义(P<0.01)。结论:桑菊饮联合诱导排痰技术治疗后可以快速有效地减轻呼吸道炎症反应,缓解临床症状,为中西医结合治疗小儿风热咳嗽的有效临床治疗方案之一。
Objective: To evaluate the clinical curative effect of sputum technique in the treatment of wind heat cough induced and its mechanism of action by Sang Juyin. Method: The 90 cases of wind heat cough were divided into sputum induction group(45 cases) and Chinese Medicine+sputum induction group(45 cases).Two groups were given hypertonic saline atomization sputum, Chinese Medicine+sputum induction group added Sang Junyin. Observe changes in symptoms and signs of children pretherapy and post-treatment 7 days, meanwhile count sputum specimen inflammatory cells respectively,examine the content of IL-6 in the induced sputum supernatant by ELISA. Result: Compare to sputum induction group, the cough, throat swelling, sleep quality, symptom of runny nose,the degree of crackles, furred tongue, pulse condition improved significantly, had significant differences(P〈0.01). After the therapy,compare to sputum induction group, the sick kids' sputum neutrocyte proportion, the proportion of eosinophils, the proportion of macrophages of the medicine+sputum induction group were significantly reduced,the interleukin-6( IL-6) in induced sputum supernate was reduce significantly, the difference was statistically significant(P〈0.01). Conclusion: Upon the treatment clinical of observation of combination of induced sputum technique,respiratory inflammation can reduce effectively, ameliorate the clinical syndromes, it's one of the best clinical treatment regimens in the treatment of wind heat cough combined with traditional Chinese and Western Medicine.
