高脱位成人髋关节发育不良患者身高与髋臼形态的相关性分析 被引量:2

摘要 目的分析高脱位髋关节发育不良(developmental dysplasia of hip,DDH)患者的身高与真臼大小、形态及后壁骨量的相关性。方法收集2014年4月-2015年5月15例(21髋)Crowe分型为Ⅳ型且继发髋关节骨性关节炎于我院行人工髋关节置换术(total hip arthroplasty,THA)治疗的DDH患者,男2例2髋,女13例19髋,平均年龄(25±7.2)岁。术前行骨盆正位X线和髋臼及股骨中上段CT扫描,分析患者身高与骨盆高度、真臼前后径宽度、深度、内壁厚度和后壁骨量的相关性。结果 CroweⅣ型DDH患者身高与骨盆高度呈正相关(r=0.723,P=0.000);身高与真臼前后径(r=0.049,P=0.834)、内壁厚度(r=0.395,P=0.076)、深度(r=0.205,P=0.373)、后壁骨量(r=0.152,P=0.511)、髋臼假体直径(r=0.010,P=0.966)均无相关性。结论 CroweⅣ型DDH患者的身高与骨盆高度呈正相关,但与真臼大小、形态及后壁骨量无相关性,身高、体型不同的患者真臼形态差异小。 Objective To analyze the correlation of height and acetabular morphology in adults with Crowe typeⅣdevelopmental dysplasia of hip(DDH). Methods From April 2014 to May 2015, 2 males(2 hips) and 13 females(19 hips) with an average age of(25±7.2) years old who were diagnosed as Crowe typeⅣdevelopmental dysplasia and had undergone total hip arthroplasty(THA) surgery in Chinese PLA General Hospital were enrolled in this study. Through standard hip X-ray and CT, the correlation of pelvic height, true acetabular anterior-posterior diameter, acetabular depth, medial wall thickness and bone stock of posterior columns was analyzed and measured. Results There was a positive correlation between height and pelvic height(r=0.723, P=0.000). No statistically significant difference was found in the correlation of height and anterior-posterior diameter(r=0.049, P=0.834), medial wall thickness(r=0.395, P=0.076), acetabular depth(r=0.205, P=0.373), bone stock of posterior columns(r=0.152, P=0.511), diameter of acetabular component(r=0.010, P=0.966). Conclusion Height is positively correlated with pelvic height in patients with Crowe typeⅣDDH. However, it is not related with acetabular morphology and bone stock of posterior columns. There are small differences between different statural patients in acetabular morphology.
