不同疾病的重症患者营养治疗评估指标的应用

Evaluation of Nutritional Therapy in Patients with Different Diseases

目的分析不同疾病的重症患者营养治疗评估指标的应用。方法选取2014年7月至2015年5月间我院收治的重症患者共186例,根据重症疾病不同将患者分为肿瘤重症组和严重慢性阻塞性肺病组。对所有患者行营养支持治疗后比较两组患者治疗期间并发症发生情况、营养治疗前后两组营养、机械通气时间以及营养达标时间等。结果两组患者均有不同程度的并发症发生情况,其中以腹胀以及恶心呕吐的发生率较大,均在10%左右;肿瘤组和肺病组营养治疗前营养指标差异不显著(P>0.05);肿瘤组和肺病组营养支持治疗后营养指标,如:血红蛋白、血清白蛋白、前白蛋白、视黄醇结合蛋白均得到提高,治疗前后比较差异显著(P<0.05);两组患者机械通气时间、住院时间以及营养达标时间上对比相差不大,差异不具有统计学意义(P>0.05)。结论对重症患者行营养支持治疗能够有效维持患者日常所需能量,而营养治疗评估指标能准确计算患者能量消耗,从而尽早达到重症患者能量供应目标。

Objective To evaluate the application of nutrition therapy in patients with severe diseases. Methods186 cases of critically ill patients were divided into severe cancer patient group and severe chronic obstructive pulmonary disease patient group according to severe of disease. All patients were underwent nutrition support therapy. The complications occur after treatment,the nutrient nutrition before and after treatment,mechanical ventilation and nutritional standard time in all patients were analyzed. Results The patients had varying degrees of complication occurrence,which were about 10% patients with bloating and greater incidence of nausea and vomiting. There was no significantly difference on nutrition indicators before treatment in between two groups（ P〈0. 05）. The nutrition indicators,such as hemoglobin,serum albumin,prealbumin,retinol binding protein,albumin indicators had improved after nutritional therapy in both groups,and had significantly different compared with that of before treatment（ P〈0. 05）. There was no significant difference on mechanical ventilation,length of hospital stay and nutritional standard time between two groups（ P〉0. 05）. Conclusion Nutritional support for critically ill patient＇s treatment can effectively maintain the daily energy of patients. The nutritional therapy evaluation index can accurately calculate the energy consumption of patients,so as to achieve the target as soon as possible.
